    양지선 2026-09-11 나는 문학에 관심이 많다. 그런데 학창 시절의 나를 생각하면 조금 뜻밖의 일이다. 그때 나는 문학보다 과학 전집을 더 좋아했다. 문학이라야 국어 교과서에 실린 작품들을 읽는 정도였다. 이유는 단순했다. 나는 1+1=2처럼 답이 분명한 것을 좋아했다. 수학에서도 실수가 허수보다 좋았다. 손에 잡히지 않고, 명료하게 설명되지 않는 것은 어딘지 불편했다. 지금은 그렇지 않다. 이제는 허수도 제법 좋아한다. 어쩌면 정신분석을 공부하게 된 일도 이 변화와 무관하지 않을 것이다. 사람의 마음에는 1+1=2로는 설명되지 않는 것이 너무나 많다는 사실을 조금씩 알게 되었기 때문이다. 문학에 관심을 갖게 된 계기는 다소 우연했다. 어느 날 시를 몇 편 써놓고 문득 궁금해졌다. ‘이게 시가 되나?’ 그래서 한 문예지에 보내보았고, 뜻밖에도 신인으로 등단하게 되었다. 돌이켜보면 그것은 내게 하나의 ‘사건’이었다. 그 일이 없었다면 나는 시를 이렇게 오래 읽고 쓰지 않았을지도 모른다. 시를 계속 쓰다 보니 다른 사람의 시를 읽게 되었고, 읽다 보니 점점 재미있어졌다. 그러면서 어느 순간부터 시와 정신분석이 이상하리만큼 닮아 있다는 느낌이 들기 시작했다. 둘 다 말해진 것보다 말해지지 않은 것을 향한다. 둘 다 한 사람이 의식적으로 설명할 수 있는 내용만을 믿지 않는다. 그리고 무엇보다 둘 다, 아직 말이 되지 않은 어떤 것이 말이 되어가는 순간을 기다린다. 얼마 전 소설가 마릴린 로빈슨(Marilynne Robinson)에 관한 글을 읽다가 무의식과 창작에 관한 몇 문장을 만났다. 로빈슨은 미국의 소설가이자 에세이스트로, 인간의 내면과 신앙, 가족과 공동체를 오랫동안 탐구해온 작가이다. 『길리아드(Gilead)』, 『홈(Home)』, 『릴라(Lila)』 등의 작품이 잘 알려져 있으며, 『길리아드』로 2005년 퓰리처상 소설 부문을 수상했다. 그녀의 글을 읽으며 특히 오래 마음에 남은 것은, 창작이 반드시 작가가 책상 앞에 앉아 의식적으로 생각하고 있는 순간에만 이루어지는 것은 아니라는 생각이었다. 어떤 문제를 오래 붙잡고 있다가 내려놓았는데, 며칠 뒤 전혀 예상하지 못한 순간에 문장이 떠오른 경험이 있다. 쓰이지 않던 글을 잠시 덮어두었다가 다시 펼쳤을 때 이전에는 보이지 않던 것이 갑자기 보이기도 한다. 애써 생각할 때에는 나오지 않던 말이 산책을 하거나 잠들기 직전, 혹은 다른 일을 하고 있을 때 문득 모습을 드러낸다. 마치 내가 생각을 멈춘 동안에도 마음의 어딘가에서는 일이 계속되고 있었던 것처럼 느껴진다. 물론 정신분석에서 말하는 무의식을 단순히 ‘의식이 쉬는 동안 대신 문제를 풀어주는 두뇌의 작업장’이라고 생각할 수는 없다. 정신분석적 무의식은 훨씬 낯설고 복잡하다. 그곳에는 우리가 알고 싶지 않아 했던 것과 아직 알 수 없었던 것이 함께 있다. 욕망과 두려움, 기억과 환상, 대상과의 관계에서 남은 흔적들이 서로 뒤섞인다. 하나의 표상이 다른 표상과 결합하고, 어떤 것은 응축되고 어떤 것은 치환된다. 과거의 경험이 현재의 감정 속으로 스며들고, 한때 말이 될 수 없었던 것이 이미지나 꿈, 행동, 신체감각, 혹은 뜻밖의 한 단어가 되어 돌아온다. 그러므로 무의식의 ‘작업’은 정답을 찾아내는 계산이라기보다 변형에 가깝다. 마음은 자신이 경험한 것을 그대로 보관하지 않는다. 끊임없이 다시 배열하고, 연결하고, 해체하고, 새로운 형태로 만들어낸다. 나는 바로 이 지점에서 시와 정신분석이 만난다고 생각한다. 시를 쓸 때 처음부터 자신이 무엇을 쓰려는지 정확히 알고 시작하는 경우는 오히려 드물다. 어떤 이미지 하나, 이상하게 마음에 걸리는 단어 하나, 설명할 수 없는 리듬이나 감각에서 문장이 시작된다. 처음에는 그것이 무엇을 의미하는지 시인 자신도 모른다. 그런데 쓰다 보면 그것들이 서로를 불러낸다. 한 단어가 오래전의 장면을 데려오고, 한 이미지가 잊고 있던 감정을 불러내며, 서로 관계없어 보이던 기억들이 어느 순간 하나의 문장 안에서 만난다. 그리고 뒤늦게 시인은 자신이 무엇을 쓰고 있었는지를 발견한다. 나는 이것이 정신분석적 장면과도 닮아 있다고 느낀다. 분석에서도 환자가 자신의 마음을 모두 알고 와서 그것을 설명하는 것은 아니다. 오히려 분석실에서는 자신도 왜 말했는지 모르는 한마디, 엉뚱하게 떠오른 기억, 반복해서 등장하는 이미지, 말하다가 갑자기 생긴 침묵 같은 것들이 중요해진다. 분석가는 그것들을 서둘러 설명하기보다 곁에 두고 바라본다. 당장 뜻을 정하지 않은 채 함께 머무른다. 그러다 보면 처음에는 서로 무관해 보였던 것들 사이에 연결이 생긴다. 한때 행동으로만 표현되던 것이 감정이 되고, 감정이 이미지가 되고, 이미지가 말이 된다. 그리고 비로소 자신의 경험에 대해 이전과는 조금 다른 방식으로 생각할 수 있게 된다. 치료가 언제나 새로운 무엇을 ‘가르치는’ 과정만은 아닌 이유도 여기에 있을 것이다. 때로 치료자는 환자의 마음속에 전혀 없던 것을 넣어주는 사람이 아니라, 이미 그 안에서 일어나고 있었으나 아직 형태를 얻지 못한 작업이 모습을 드러낼 수 있도록 기다려주는 사람에 가깝다. 무언가가 말이 되기까지 필요한 시간. 이해가 도착하기까지 필요한 침묵. 서로 연결되지 못했던 경험들이 만나기까지 필요한 공간. 정신분석은 어쩌면 그 시간과 공간을 제공하는 작업인지도 모른다. 그리고 시 역시 그러하다. 시인이 찾아낸 단어는 무의식 깊은 곳에 완성된 채 묻혀 있던 ‘진실’을 그대로 끌어올린 것이라기보다, 아직 말이 없던 경험이 언어와 만나는 순간에 새롭게 만들어진 것에 가까울 것이다. 언어가 경험을 표현하는 동시에 경험 자체를 변형시키기 때문이다. 그래서 어떤 문장은 쓰고 난 뒤에야 자신이 무엇을 느끼고 있었는지 알게 한다. 어떤 시는 시인이 알고 있던 것을 적은 것이 아니라, 쓰는 과정에서 비로소 알게 된 것을 담는다. 환자에게도 비슷한 순간이 있다. 오랫동안 반복하던 행동에 처음으로 이름을 붙이는 순간, 모호한 불편함이 하나의 감정으로 느껴지는 순간, 자신에게 일어난 일을 이전과 다른 언어로 이야기하게 되는 순간. 그때 경험은 단순히 ‘설명된 것’이 아니라 조금 다른 것이 된다. 말은 과거를 바꾸지는 못하지만, 과거가 마음속에 존재하는 방식을 바꿀 수 있다. 그것이 시의 작업이면서 동시에 분석의 작업일지도 모른다. 학창 시절의 나는 분명한 것을 좋아했다. 1+1은 반드시 2여야 마음이 편했다. 그러나 사람의 마음을 오래 들여다보다 보면, 알 수 없는 것을 알 수 없는 상태로 견뎌야 하는 시간이 있다는 것을 배우게 된다. 너무 빨리 이해하려 하면 오히려 중요한 것을 잃어버릴 수도 있다. 시 앞에서도 그렇고, 환자의 마음 앞에서도 그렇다. 때로는 이해하려 애쓰는 것보다 조금 더 오래 머무르는 일이 필요하다. 아직 이름 붙일 수 없는 감각과 생각을 성급히 설명하지 않고 두는 일. 내가 아직 모른다는 사실을 견디면서 그 안에서 무엇이 생겨나는지를 기다리는 일. 아마도 나는 문학을 읽고 정신분석을 공부하면서, 조금씩 허수를 좋아하게 된 것 같다. 눈앞에 분명히 보이는 것만이 존재하는 것은 아니라는 사실을 알게 되었으므로. 그리고 우리가 알지 못하는 동안에도 마음은 가만히 있지 않는다는 것을 이제는 조금 믿게 되었으므로. 우리 안의 어떤 경험들은 오래도록 말이 되지 않은 채 머문다. 그러나 충분한 시간과 공간이 주어진다면, 어느 날 그것은 꿈이 되고, 이미지가 되고, 한마디 말이 되고, 때로는 한 편의 시가 된다. 정신분석가가 하는 일 역시 어쩌면 그 순간을 함께 기다리는 것인지 모른다. 아직 알지 못하는 것이,

    이창훈 2026-08-16 서론 프로이트가 정신분석을 창시한 이래 약 140년 동안, 정신분석은 심리적의학적 치료법인 동시에 인간의 마음을 이해하기 위한 학문으로, 더 나아가 현대 문화와 사상에 깊은 영향을 미친 인류의 자산으로 자리매김했다. 오늘날 정신분석의 영향은 정신의학과 심리학의 영역에 국한되지 않는다. 문학, 예술, 철학, 종교, 역사와 사회 현상에 대한 연구에 이르기까지 인간의 삶과 문화를 이해하고자 하는 다양한 분야에서 정신분석적 개념과 사유의 흔적을 찾아볼 수 있다. 이러한 확장은 정신분석이 이룬 중요한 성취이다. 서로 다른 학문적·전문적 배경을 가진 사람들이 정신분석을 자신의 영역에 접목함에 따라, 정신분석은 새로운 질문과 관점을 지속적으로 받아들이게 되었다. 정신분석을 문학과 예술, 역사와 사회, 종교와 문화의 이해에 적용하는 연구뿐 아니라, 최근에는 신경과학, 과학기술, 인공지능과 같은 새로운 영역과의 접점도 활발히 탐색되고 있다. 정신분석의 이러한 다학제성과 개방성은 현대 정신분석이 유지하고 발전시켜야 할 중요한 가치이다. 그렇다면 여기에서 한 가지 질문을 제기할 수 있다. 정신과 의사들이 자신들의 전문적 배경과 임상 경험에 기초해 정신분석을 연구하고 훈련하는 독자적인 학술 공동체를 구성하는 것은 어떤 의미를 갖는가? 정신분석이 인간과 문화에 관한 일반 이론으로 확장되어 온 것과 별개로, 정신분석이 히스테리 증상의 의학적 치료에서 출발하였으며 정신의학의 역사와 긴밀한관계를 맺어 왔다는 사실은 부정하기 어렵다. 또한 정신의학의 전문가인 정신과 의사들이 정신분석과 정신의학의 접점에특별한 관심을 기울이고, 그들 고유의 임상 경험을 공유하며, 이를 토대로 연구, 교육, 훈련을 수행하는 공동체를 형성하는것은 그 자체로 자연스러운 일이다. 그러나 오늘날 정신분석계에서 이러한 주장을 제기하는 일은 단순하지 않다. 정신분석의 역사에서 직역에 대한 자격 제한은 오랫동안 논쟁의 중심에 있었다. 특히 정신분석가가 되기 위한 훈련과정에 지원할 자격을 의사에게만 허용했던 과거 미국정신분석협회의 제도는 현대에 이르러 정당한 비판과 도전에 직면하였다. 따라서 정신과 의사들만의 정신분석 학술단체또는 훈련체계의 필요성을 논의하는 것을 배타적 의료주의로 회귀하려는 주장과는 명확하게 구분할 필요가 있다. 본 논문이 주장하고자 하는 것은 정신분석가가 될 수 있는 자격을 특정 직역에 독점적으로 부여해야 한다는 것이 아니다. 정신분석의 다학제적 발전과 직업적 배경에 따른 비차별의 원칙을 존중하면서도, 정신과 의사만으로 구성된 독자적인 정신분석 학술단체와 정신과 의사를 위한 고유한 분석가 훈련체계가 필요하다는 것이 본 논문의 핵심 주장이다. 정신과 의사는 의학과 정신의학 훈련을 통해 형성된 고유의 임상적 책임과 관점을 지니며, 이러한 전문적 기반은 정신분석가가 되는 과정에서 정신분석적 정체성과 통합되어야 한다. 다양성의 확대가 각 전문 분야의 고유성을 희석시키는 방식으로 이루어져서는안 된다. 진정한 다양성은 서로 다른 전문적 배경에서 비롯된 차이와 고유성을 인정하고, 각 전문적 전통 아래에서 자신의정체성을 유지하면서 상호 대화할 수 있을 때 가능하다. 정신분석의 개방과 비차별의 역사적 의미 이 문제는 1980년대 미국 정신분석계에서 벌어진 비의사 정신분석가 훈련을 둘러싼 논쟁을 떠올리게 한다. 미국정신분석협회(American Psychoanalytic Association, APsA)는 오랫동안 정신분석 훈련을 의사 중심으로 운영해왔으나, 이러한 정책은 심리학자를 포함한 다른 전문 분야의 참여를 제한한다는 비판을 받았다. Agrawal(2024)은 APsA가 1911년 창립 이후 1989년까지 회원자격을 의사에게 제한하였으며 , 1980년대의법적 분쟁을 거쳐 1989년부터 비의사에게도 의사와 동일한 기준으로 정신분석 훈련의 문호를 개방하게 되었다고 정리한다. 비의사에게 정신분석 훈련을 개방하면서 정신분석의 이론적·임상적 발전에는 새로운 가능성이 열렸고, 동시에 큰 도전이시작되었다. 정신과 의사이자 정신분석가로서 APsA 회장(1980-81)을 역임한 Cooper(1990)는, 미국정신분석의 탈의료화가본격화되던 시기에 이것이 정신분석의 전문적 정체성과 정신의학과의 관계에 미칠 영향을 일찍이 주목하였다. 그는 정신분석의 역사에서 의사가 아닌 많은 창조적 인물들이 중요한 역할을 해왔음을 인정하면서도, 자신이 우려하는 것은 비의사 분석가의 존재가 아니라, 미국에서”정신의학적 정신분석(medical psychoanalysis)”이라는 독자적인 전문적 조직적 정체성이소멸할 때 나타날 결과라고 하였다. Cooper는 특히 의학적 인프라와의 단절이 가져올 ‘연구의 위기’를 경고했다. 그는 정신분석이 의과대학 및 대학병원의 체계적인 연구 시스템에서 소외될 경우, 객관적 데이터에 기반한 임상 연구보다는 인문학적 해석에 치중하는 ‘연성 연구(soft research)’로 흐를 위험이 크다고 보았다. 이는 결과적으로 현대 의학이 요구하는 증거 기간의 치료 효과 입증을 어렵게 만들어, 보건 정책이나 보험 체계 내에서 정신분석의 학문적 신뢰성과 입지를 약화시키는 결과를 초래할 수 있다는 것이다. 따라서 정신분석의 개방성과 다학제성을 인정하는 것과 정신의학적 정신분석의 고유한 전통과 역할을 보존하는 것은 서로 배타적인 과제가 아니다. Sabshin(1985) 역시 당시 비의사 후보자에 대한 개방을 둘러싼 논쟁을 단순한 찬반의 문제로 보지 않았다. 그는 비의사 후보자에게 정신분석 수련을 폭넓게 개방하는 변화가 장기적으로 정신분석과 정신의학의 유대를 약화시킬 가능성을 우려했다. 정신분석이 더 이상 의료분야와 긴밀히 연결된 전문영역으로 인식되지 않을 경우, 의사들이 정신분석 훈련을 선택할 동기역시 약화될 수 있다는 것이 그의 문제의식이었다. 그는 정신분석과 정신의학의 관계가 약화될 경우 양쪽 모두 중요한 것을 잃을 수 있다고 주장하면서, 두 분야가 결별하기 보다는 현실의 변화에 맞추어 관계를 새롭게 구성해야한다고 했다. 그의 관점에서 중요한 것은 정신분석을 정신의학의 하위영역으로 환원하는 것도, 반대로 정신의학과의 연결을 끊은 것도 아니었다. 오히려 진단, 치료 선택, 약물 치료와 정신 치료의 통합, 신경과학과의 관계 등 임상 현실에서 정신의학과 정신분석이 서로에게 제공할 수 있는 고유한 자원을 인정하는 것이었다. 비차별의 역사적 성취는 충분히 인정되어야 한다. 정신분석가로서의 자격과 가치를 출신 직역에 따라 위계화해서는 안되며, 정신과 의사, 임상심리학자, 사회복지사, 인문학자 등 다양한 배경의 전문가들이 정신분석에 기여할 수 있어야 한다. 그러나 직역에 따른 차별을 없애는 것과 서로 다른 전문적 배경에서 비롯되는 차이를 인정하지 않는 것은 동일한 문제가 아니다. 각 전문 분야는 서로 다른 교육과 임상 경험, 책임의 범위 속에서 정신분석에 접근한다. 이러한 차이를 인정하는 것은 차별이 아니라, 오히려 다양성 시대에 정신분석의 성숙을 위한 전제일 수 있다. 비차별 정책의 성취와 역설 정신분석계의 비차별 정책은 다양한 전문 분야 출신의 정신분석가가 등장할 수 있는 제도적 기반을 마련했다. 이 과정에서정신분석은 의료 전문영역에만 머무르지 않고 인간과 문화에 관한 보다 폭넓은 학문으로 발전했다. 그러나 다양성의 확대에는 또 다른 과제와 도전이 뒤따른다. 다양성을 강조하는 과정에서, 역설적으로, 어느 한 전문 분야가 지닌 고유한 경험과관점을 드러내기 어려운 환경이 되어버린다면, 오히려 다양성의 기반 자체가 약화될 수 있다. 비차별(Non-discrimination)은 차이의 부정(Non-differentiation)을 의미하지 않는다. 정신과 의사에게 정신분석은 인간의 무의식과 욕망을 이해하기 위한 이론인 동시에 정신질환으로 고통받는 환자를 이해하고 치료하기 위한 임상적 도구이다. 정신과 의사는 정신병리와 진단, 신체질환과 정신질환의 감별, 약물치료, 자살 및 응급상황의 평가와 개입, 다양한 치료방법 사이의 선택과 통합에 대한 책임을 진다. Sabshin(1985)은 바로 이러한 진단과 치료적 분류의 문제를 정신의학과 정신분석의관계에서 핵심적인 쟁점으로 보았다. 정신분석적 기법의 능숙함과 별개로, 어떤 환자가 정신분석에 적합한지, 정신치료가더 적절한지, 약물치료나 다른 의학적 개입이 필요한지를 판단하는 것은 임상적 책임의 중요한 일부이기 때문이다. 정신과 의사 출신 정신분석가의 이러한 특수성은 실제 전문단체의 역사에서도 확인된다. American College of Psychoanalysts는 1969년 정신과 의사 출신 정신분석가들을 위한 조직으로 설립되었다(Clemens 2016). 이 단체는 정신과 의사의 정신분석적 실천이 의과대학과 전공의 수련을 통해 형성된 진단적 관점, 마음과 몸의 관계에 대한 관심, 정신약물학과정신분석의 통합, 그리고 경험적 연구에 대한 친숙성이라는 고유한 특징을 갖는다고 보았다. 동시에 심리학자와 사회복지사 출신 정신분석가들이 자신의 전문조직을 갖는 것처럼 정신과 의사 출신 정신분석가가 고유한 공동체를 갖는 것은 정신분석 전체의 다양성과 양립 가능하다고 명시한다. 이는 전문적 고유성을 보존하는 것이 다른 직역을 배제하거나 위계화하는 것과 동일하지 않음을 보여주는 구체적인 사례이다. 따라서 정신분석의 다학제성을 발전시키는 것과 각 전문 분야가 자신의 고유한 임상적 학문적 전통 속에서 정신분석을 발전시키는 것은 양립할 수 있다. 정신과 의사 역시 자신의 의학적 정체성을 포기하지 않고, 정신과 의사이면서 동시에 정신분석가라는 두 전문적 정체성을 통합할 수 있어야 한다. 이러한 통합이 개인에게만 맡겨져서는 안되며, 교육과 제도 안에서 지속적으로 이루어지려면, 정신과 의사들이 자신의 임상적 경험을 공유하고 연구할 학술 단체 뿐 아니라 그 고유 전문성을 정신분석가 형성과정에 반영하는 독자적 훈련체계가 필요하다. 정신과 의사의 정신분석에 대한 정체성의 위기 미국 정신분석의 탈의료화와 정신과 의사 지원자의 감소는 이 문제를 상징적으로 보여준다. Cooper(1990)에 따르면 1978-1979년부터 1987-1988년까지 약10년 동안 미국의 정신분석 훈련 전체 지원자가 30% 감소하였고, 이중 비의사 지원자는 두배로 증가했고 의사 지원자는 약 50% 감소하였다. 이 수치를 단순히 비차별 정책의 결과라고 단정해서는 안 되지만, Cooper는 이를 정신분석의 ‘탈의료화(demedicalization)’라는 보다 광범위한 변화와 연관지어 해석하였다. 그는 정신분석이 더 이상의학적 전문영역으로 인식되지 않을수록, 오랜 의학교육과 정신과 수련을 마친 젊은 정신과 의사들이 기존 의학교육과의연속성을 느끼기 힘든 정신분석 훈련을 선택하기가 더욱 어려워진다고 우려하였다. Cooper(1990)의 우려는 단순히 정신과 의사 수의 감소에 그치지 않는다. 그는 의사 출신 정신분석가들이 줄어들수록 정신분석이 의과대학 정신의학과 전공의 교육, 임상 연구와 점차 멀어질 수 있으며, 그 결과 정신분석의 임상적과학적 기반도 약화될 수 있다고 예견하였다. 특히 정신분석 연구는 대학과 의과대학의 연구 인프라, 임상적 자원, 과학적 훈련과의 연결을필요로 하므로, 정신의학과의 관계 단절은 정신분석의 연구 역량에서 영향을 미칠 수 있다는 것이다. Kernberg(2006) 역시 정신분석 교육기관이 정신의학으로부터 멀어지는 것을 강하게 비판하였다. 그는 그 결과 새로운 정동연구, 생물학적 정신의학과의 대화에 제약이 생겼고, 이런 지적 고립은 정신분석의 창조성과 과학적 발전의 저해를 가져왔다고 했다. 오늘날 정신과 의사와 정신분석 사이의 거리는 정신의학 내부의 변화와도 관련이 있다. 현대 정신의학은 생물학적 의학, 정신약물학, 신경과학 및 근거중심의학을 중심으로 빠르게 발전해 왔다. 이러한 발전으로 정신질환의 치료와 연구에 중요한성취를 이루었지만, 환자의 주관적 경험과 무의식적 의미, 관계적 맥락을 깊이 탐색하는 전통이 상대적으로 약화되는 부작용을 낳았다. Sabshin(1998)은 미국 정신과 의사들 사이에서 ‘심리적 이해 역량(psychological mindedness)’이 감소하고 있다고 우려했다. 그가 말한 심리적 이해 역량이란 아동기 경험, 주요 정동, 방어, 성격구조, 전이와 같은 심리적 과정들 사이의의미와 인과적 연결을 파악하는 능력이다. 그는 이 능력이 생물학적 정신의학과 대립되는 것이 아니라, 이를 보완하는 것이며, 정신분석가들이 의과대학과 정신과 전공의 교육에 적극적으로 참여해야 한다고 강조한다. 이 문제는 최근에도 현재진행형이다. Kernberg(1986)는 정신분석 교육이 개방적인 과학적 탐구보다 교조적 전수에 가까워질 위험을 지적하면서, 정신분석 연구기관이 ‘신학교’나 폐쇄적 기술학교가 아니라 지식을 전달하고 탐구하며 새로운 지식을 생산하는 ‘대학형’ 기관으로 기능해야 한다고 주장하였다. 그의 모델에서 정신분석 교육은 특정 학파의 권위를 반복하는 것이 아니라 서로 다른 이론과 기법을 비판적으로 검토하고, 최신 임상 및 과학적 지식을 수용하며, 치료자로서의 기술과 연구자로서의 태도를 함께 발전시키는 과정이다. 이러한 관점은 정신과 의사들만의 정신분석 공동체 역시 단순한 직역 보호단체가 아니라, 정신의학이라는 전문적 기반 위에서 정신분석을 비판적으로 연구하고 발전시키는 학술 공동체이어야 함을 시사한다. Kernberg(2006)는 정신분석 교육의 권위주의, 과도한 위계화, 연구의 경시, 외부 학문과의 단절이 수련생의 창조성과 전문적성장을 저해할 수 있다고 다시 강조한다. 특히 그는 정신분석가의 정체성을 보호한다는 이유로 정신분석적 정신치료, 집단치료, 부부치료, 신경과학과 같은 새로운 관심을 배척하는 것은 정체성을 지키는 것이 아니라 오히려 두려움에 기반한 보수주의일 수 있다고 보았다. 이 점은 정신과 의사 출신 정신분석가들에게 특히 중요한 의미를 가진다. 정신과 의사가 정신분석가가 되었다고 해서 정신의학적 진단, 약물치료, 신경과학, 의료윤리와 응급상황에 대한 책임을 내려놓는 것이 아니다. 분석가 훈련은 기존의 의학적전문성에 무의식, 전이, 역전이, 방어, 관계적 의미에 대한 깊은 이해를 더해 하나의 임상적 정체성으로 통합하는 과정이어야 한다. 이러한 목적을 지속적으로 구현하기 위해서는 정신과 의사의 임상적 현실과 책임을 공유하는 훈련 분석가(training analyst)와 지도감독자(supervising analyst), 교육과정, 동료집단을 갖춘 훈련체계가 필요하다. 치료법으로서 정신분석의 정체성과 현대 정신의학 정신분석은 인간과 문화에 관한 이론이기 이전에 임상적 만남에서 출발하였다. 프로이트의 발견 역시 히스테리 환자의 증상을 이해하고 치료하려는 과정에서 형성되었으며, 이후 정신분석은 인간의 마음에 관한 폭넓은 이론으로 발전했지만 치료적 실천은 여전히 정신분석의 핵심 토대 가운데 하나이다. 따라서 현대 정신분석이 다양한 학문과 문화 영역으로 확장된다고 하더라도, 정신질환으로 고통받는 환자를 어떻게 이해하고 치료할 것인가라는 질문을 포기할 수 없다. 특히 현대의 정신과 임상은 고전적 정신분석이 주로 다루었던 신경증적 환자보다 훨씬 폭넓은 환자군을 마주한다. 심각한인격병리, 자해와 자살 위험, 양극성 장애와 정신병적 상태, 발달장애, 신체질환과 정신질환이 복합된 환자들이 일상적인 정신과 진료의 대상이다. 이러한 환자를 치료하는 과정에서 정신과 의사는 정신역동적 이해와 함께 진단적 판단, 위험도 평가, 약물 치료, 치료 환경 조정, 현실적인 치료 계획을 동시에 수행해야 한다. Sabshin(1998)은 정신과 의사가 정신치료 능력을 유지하는 것이 정신분석뿐 아니라 정신의학의 미래 정체성을 결정짓는 핵심 요소라고 보았다. 그는 정신과 의사가 심리적 이해 역량(psychological mindedness)과 분석적 기술을 평가절하하고 소홀히할 경우, 정신의학은 인간의 주관적 경험을 다루는 학문적 주도권을 잃고 단순히 ‘약물 처방가’에 머물 수 있다고 지적했다. 이는 정신의학이 생물학적 치료와 심리적 이해를 통합해 전인적 치유를 제공하는 독자적인 전문적 정체성을 스스로 포기하게 된다는 뜻이다. 또한 이러한 경우, 정신분석 역시 근본적으로 흔들릴 수 있다. 생물학적 치료와 정신치료를 통합하는 정신과 의사의 리더십과 영향력을 잃게 된다면, 정신분석에서 ‘의학적 엄밀함’이 희석되고 과학으로서의 정체성을 유지하기 어려워지기 때문이다. 이처럼 약물치료와 정신치료를 통합하는 정신과 의사의 시각과 기술, 그리고 임상 경험은 비의사 분석가와 구분되는 중요한 특성이다. 이 영역에서 필요한 임상적·이론적 연구에 정신과 의사 출신 정신분석가들의 기여가 절실하며, 그들이 고유한임상적 리더십을 발휘할 수 있는 시스템이 필요하다. Kernberg(2007)는 이러한 현대 임상 현실을 정신분석 교육이 적극적으로 수용해야 한다고 주장했다. 그는 정신분석적 정신치료를 정신분석 훈련에서 배제하거나 부차적인 것으로 취급하는 전통이 실제 임상에서 만나는 다수의 환자에게 필요한 치료 역량을 충분히 길러주지 못한다고 비판하였다. 표준적 정신분석과 정신분석적 정신치료의 적응증과 금기, 공통점과 차이, 치료기법을 높은 수준에서 함께 가르치는 것이 오히려 정신분석 기법의 특수성을 더 명확하게 이해하게 하고, 임상가가폭넓은 환자군에 정신분석적 지식을 적용할 수 있게 한다는 것이다. Kernberg(2007)는 또한 정신분석 교육기관이 대학, 특히 정신의학과 심리학과의 연결을 회복하고, 정상 및 병적 정신발달, 정신병리, 치료기법뿐 아니라 신경과학과 정신약물학 등 인접 분야의 발전과 적극적으로 교류해야 한다고 강조하였다. 연구는 이 연결의 핵심이다. 정신분석이 임상적 경험과 해석적 전통 안에 고립될 것이 아니라, 임상과 자연주의 역사적해석학적 연구와 경험적 연구를 폭넓게 발전시키고, 다른 학문과 협력하여 자신의 가설을 비판적으로 검토해야 한다는 것이다. 이러한 관점에서 정신과 의사 중심의 정신분석 학술단체와 분석가 훈련체계는 분리될 수 없다. 학술단체는 정신분석과 현대정신의학, 정신약물학, 신경과학, 정신치료 연구를 연결하는 지적 기반을 제공하고, 훈련체계는 그러한 지식과 임상적 태도가 다음 세대의 정신과 의사 분석가에게 실제로 전승되도록 한다. 두 제도가 함께 작동할 때 정신과 의사는 정신분석적 심층성을 유지하면서 현대정신의학의 발전을 수용할 수 있고, 정신분석 역시 실제 정신병리와 치료 현장에서 멀어지지 않을수 있다. 정신과 의사만의 정신분석 학술단체와 분석가 훈련이 필요한 이유 이제 본 논문의 핵심 질문으로 돌아갈 수 있다. 왜 정신과 의사만으로 구성된 정신분석 학술단체와 정신과 의사를 위한 분석가 훈련 체계가 필요한가? 첫째, 정신과 의사의 의학적 책임과 정신분석적 사고를 지속적으로 통합하기 위해서이다. 정신과 의사는 환자의 무의식과전이를 이해하는 동시에 정신병리, 신체질환, 약물치료, 자살 위험과 응급상황을 평가하고 다양한 치료법 가운데 적절한 선택을 해야 한다. 이런 복합적 책임이 정신분석 치료 과정에서 어떤 의미를 갖는지 연구하고 교육하는 것은 정신과 의사 출신분석가들에게 고유한 과제이다. 독자적 학술단체는 이 문제를 지속적으로 탐구할 장을 제공하며, 분석가 훈련체계는 이를수련생의 임상적 정체성 형성에 직접 반영할 수 있다. 둘째, 정신분석 안에서 정신의학적 관점을 유지하기 위해서이다. Cooper(1990)와 Sabshin(1985)이 우려한 것처럼 정신분석과 정신의학이 분리될 경우, 정신분석은 중증 정신병리, 진단, 약물치료, 의료체계와의 연결을 점차 잃을 위험이 있다. 반대로 정신의학 역시 무의식과 관계의 의미를 다루는 깊이를 잃을 수 있다. 정신과 의사만의 학술단체와 훈련체계는 어느 한쪽의 우월성을 주장하기 위한 것이 아니라, 두 분야의 접점을 제도적으로 유지해 상호 손실을 줄이기 위한 장치가 될 수 있다. 세째, 정신의학 안에서 정신분석적 사고와 정신치료의 전통을 보존하고 다음 세대에 전승하기 위해서이다. 앞서 논의했듯이, Sabshin(1998)이 지적한 ‘심리적 이해 역량’의 약화는 정신분석만의 위기가 아니라 정신의학의 임상적 깊이가 약화되는문제일 수 있다. 환자를 진단명과 약물 반응의 대상으로만 보지 않고, 발달사, 관계, 방어, 욕망과 갈등의 맥락에서 이해하는능력을 유지하려면 정신과 의사 분석가들이 정신과 교육에 지속적으로 참여해야 한다. 이를 위해서는 개개 분석가의 노력에 의존하기보다, 교육자와 지도감독자를 재생산할 수 있는 안정적인 분석가 훈련 기반이 필요하다. 네째, 이러한 독자적 조직과 훈련은 정신분석의 다학제성과 모순되기보다 그것을 구현하는 한 방식이 될 수 있다. Clemens(2016)가 기술한 사례는 심리학자, 사회복지사, 정신과 의사 등 서로 다른 전문적 배경을 가진 분석가들이 각자의 전문 공동체를 가지면서도 더 넓은 정신분석의 장에서 공통의 기반을 공유할 수 있음을 보여준다. 하나의 통합된 정신분석 공동체와 전문 분야별 공동체는 경쟁적 구조일 필요가 없다. 중요한 것은 각 공동체가 다른 직역을 배제하거나 위계화하지 않으면서 자신의 고유한 전문성을 발전시키고 상호 교류하는 것이다. 마지막으로, 정신과 의사에게 정신분석을 다시 현실적이고 전문적인 진로로 제시하기 위해서이다. Agrawal(2024)이 소개한Michael Balint Scholarship은 의대생에게 정신역동적정신분석적 사고를 접할 구체적인 기회를 제공하려는 최근의 시도이다. 그러나 이런 관심이 일회성 경험에 그치지 않고 정신과 전공의 교육과 이후의 분석가 양성으로 이어지려면, 정신과 의사가 의학적 정체성을 포기하지 않으면서 정신분석가로 성장할 수 있다는 실제적인 진로 모델이 필요하다. 정신과 의사를 위한 학술단체와 분석가 훈련체계는 바로 그러한 전문적 경로를 가시화하고 지속시키는 기반이 될 수 있다. 결론: 다양성과 고유성은 대립하지 않는다. 정신분석의 역사는 확장의 역사였다. 정신분석은 의학에서 출발했지만 심리학과 인문학, 예술과 사회과학을 만나면서 훨씬풍부한 지적 전통으로 발전하였다. 정신분석 교육과 회원 자격의 문호를 넓게 개방한 변화 역시 이러한 역사적 발전의 중요한 한 부분이다. 그러나 다양성을 보호한다는 것이 각 전문적 배경에서 비롯되는 차이와 고유성을 지워야 한다는 의미는 아니다. 비차별을비구분과 혼동하지 않도록 해야 한다. 서로 다른 전문적 전통을 가진 사람들이 등등한 정신분석가로 인정받는 것과, 그들이정신분석에 가져오는 서로 다른 지식과 책임, 임상 경험을 인정하는 것은 충분히 양립할 수 있다. 오히려 성숙한 다학제적공동체라면 구성원들이 자신의 전문적 정체성을 충분히 발전시키면서 서로 만나고 교류할 수 있어야 한다. 그런 의미에서 정신과 의사만으로 구성된 정신분석 학술단체와 정신과 의사를 위한 분석가 훈련체계의 존재는 정신분석의다양성에 반하는 것이 아니다. 그것은 정신분석이라는 넓은 공동체 안에서 의학적정신의학적 정신분석이라는 하나의 전문적 전통을 보존하고 발전시키는 방식이다. 그 목적은 정신과 의사가 정신분석으로부터 멀어지고 정신분석이 정신의학으로부터 멀어지는 현재의 간극을 줄이며, 두 분야 사이의 오래된 대화를 현대의 임상적과학적 조건 속에서 새롭게 이어가는 데 있다. 정신과 의사에게 정신분석은 과거 정신의학의 한 시대를 상징하는 유산에 그치지 않는다. 그것은 환자를 증상과 진단의 집합을 넘어 하나의 주체로 이해하게 하는 임상적 사고방식이며, 생물학적 치료와 심리적 치료를 한 사람 안에서 통합하게 하는 중요한 지적임상적 전통이다. 동시에 정신분석 분야에서 정신과 의사는 실제 정신병리, 의학적 진단과 치료, 신경과학과연구의 세계로 연결되는 중요한 통로가 될 수 있다. 정신분석이 다양한 전문 분야에 문호를 열어온 오늘날 필요한 질문은, 그 다양성을 구성하는 각각의 전문적 전통이 자신의고유성을 충분히 발전시킬 제도적 공간이 보장받고 있는가 하는 것이다. 정신과 의사의 경우 그 공간은 단순한 친목적 공동체를 넘어, 독자적인 학술단체와 분석가 훈련 체계라는 형태로 발전할 필요가 있다. 정신과 의사만의 정신분석 학술단체와 분석가 훈련은 이 질문에 대한 하나의 실천적 답이 될 수 있다. 이러한 제도가 개방적이고 비판적인 학문적 태도, 현대정신의학과 신경과학에 대한 관심, 정신분석적 정신치료를 포함한 폭넓은 임상 역량, 그리고 다른 전문분야의 정신분석가들과의 적극적인 교류를 유지한다면, 그것은 과거의 배타적 의료주의로의 회귀가 아니다. 오히려 정신과 의사의 의학적 전문성과 정신분석적 정체성을 통합하고, 정신의학과 정신분석의 상호교류를 다음 세대로 이어가며, 정신분석의 다학제적 다양성을 더욱 실질적이고 풍부하게 만드는 기반이 될 수 있을 것이다. 참고문헌 Agrawal, H. Introduction For approximately 140 years since Freud founded psychoanalysis, psychoanalysis has established itself not only as a psychological and medical treatment but also as a discipline for understanding the human mind and, more broadly, as an intellectual legacy that has profoundly influenced modern culture and thought. Today, the influence of psychoanalysis is not confined to psychiatry and psychology. Psychoanalytic concepts and modes of thought can be found across a wide range of fields concerned with understanding human life and culture, including literature, art, philosophy, religion, history, and the study of social phenomena. This expansion is one of psychoanalysis’s important achievements. As people from different academic and professional backgrounds have brought psychoanalysis into their own fields, psychoanalysis has continued to encounter new questions and perspectives. In addition to studies applying psychoanalysis to literature and art, history and society, religion and culture, there is now active exploration of its intersections with newer fields such as neuroscience, science and technology, and artificial intelligence. This interdisciplinarity and openness are important values that contemporary psychoanalysis should preserve and further develop. This raises an important question: What does it mean for psychiatrists to form an independent scholarly community in which they study and train in psychoanalysis on the basis of their own professional background and clinical experience? Quite apart from psychoanalysis’s expansion into a general theory of human beings and culture, it is difficult to deny that psychoanalysis originated in the medical treatment of hysterical symptoms and has maintained a close relationship with the history of psychiatry. It is therefore natural that psychiatrists, as specialists in psychiatry, should take a particular interest in the interface between psychiatry and psychoanalysis, share their distinctive clinical experiences, and build a community in which research, education, and training can be pursued on that basis. Yet making such an argument within contemporary psychoanalysis is not straightforward. Restrictions on eligibility according to professional background have long been a central controversy in the history of psychoanalysis. In particular, the former policy of the American Psychoanalytic Association that restricted eligibility for psychoanalytic training to physicians was, in the modern era, subjected to legitimate criticism and challenge. The argument for a psychoanalytic scholarly organization or training system specifically for psychiatrists must therefore be clearly distinguished from a return to exclusionary medicalism. The argument of this paper is not that eligibility to become a psychoanalyst should be monopolized by any particular profession. Rather, while respecting the interdisciplinary development of psychoanalysis and the principle of non-discrimination based on professional background, this paper argues for the need for an independent psychoanalytic scholarly organization composed of psychiatrists and for a distinctive psychoanalytic training system designed for psychiatrists. Psychiatrists have particular clinical responsibilities and perspectives formed through medical and psychiatric training, and this professional foundation should be integrated with a psychoanalytic identity in the process of becoming a psychoanalyst. The expansion of diversity should not occur by diluting the distinctiveness of individual professions. Genuine diversity becomes possible when differences and distinctiveness arising from different professional backgrounds are recognized and each professional tradition can maintain its own identity while remaining in dialogue with others. The Historical Significance of Openness and Non-Discrimination in Psychoanalysis This issue recalls the debate over the training of non-physician psychoanalysts in the United States during the 1980s. The American Psychoanalytic Association (APsA) had long organized psychoanalytic training primarily around physicians, but this policy was criticized for restricting participation by psychologists and members of other professions. Agrawal (2024) notes that, from its founding in 1911 until 1989, APsA restricted membership to physicians, and that following legal disputes in the 1980s, it opened psychoanalytic training to non-physicians in 1989 under the same standards applied to physicians. Opening psychoanalytic training to non-physicians created new possibilities for the theoretical and clinical development of psychoanalysis, while also introducing major challenges. Cooper (1990), a psychiatrist and psychoanalyst who served as president of APsA from 1980 to 1981, recognized early on that the growing demedicalization of American psychoanalysis could affect both its professional identity and its relationship with psychiatry. While acknowledging that many creative non-physicians had played important roles throughout the history of psychoanalysis, he emphasized that his concern was not the existence of non-physician analysts but the consequences that might follow if “medical psychoanalysis” disappeared as a distinct professional and organizational identity in the United States. Cooper was particularly concerned about a “research crisis” that could result from separation from the medical infrastructure. He argued that if psychoanalysis became isolated from the systematic research environments of medical schools and university hospitals, it might increasingly rely on “soft research” centered on humanistic interpretation rather than on clinical research grounded in objective data. In turn, this could make it more difficult to demonstrate therapeutic effectiveness according to the evidentiary standards of modern medicine and thereby weaken the academic credibility and position of psychoanalysis within health policy and insurance systems. Thus, recognizing the openness and interdisciplinarity of psychoanalysis and preserving the distinctive tradition and role of medical psychoanalysis are not mutually exclusive tasks. Sabshin (1985) likewise did not regard the controversy over opening training to non-physician candidates as a simple question of being for or against such a change. He was concerned that broadly opening psychoanalytic training to non-physicians might, over time, weaken the relationship between psychoanalysis and psychiatry. If psychoanalysis were no longer perceived as a professional field closely connected with medicine, physicians might also become less motivated to pursue psychoanalytic training. Sabshin argued that both psychoanalysis and psychiatry could lose something important if their relationship weakened, and that rather than separating, the two fields should reconstruct their relationship in response to changing circumstances. From his perspective, the goal was neither to reduce psychoanalysis to a subfield of psychiatry nor to sever its ties with psychiatry. Rather, it was to recognize the distinctive resources that psychiatry and psychoanalysis could offer one another in clinical practice, including diagnosis, treatment selection, the integration of pharmacotherapy and psychotherapy, and engagement with neuroscience. The historical achievement of non-discrimination should be fully acknowledged. The qualifications and value of psychoanalysts should not be ranked according to their original professions, and professionals from diverse backgrounds—including psychiatrists, clinical psychologists, social workers, and scholars in the humanities—should be able to contribute to psychoanalysis. However, eliminating discrimination based on profession is not the same as refusing to recognize differences that arise from different professional backgrounds. Each profession approaches psychoanalysis with different forms of education, clinical experience, and scope of responsibility. Recognizing these differences is not discrimination; rather, it may be a prerequisite for a mature psychoanalysis in an era of diversity. The Achievements and Paradox of Non-Discrimination The policy of non-discrimination in psychoanalysis created an institutional basis for psychoanalysts from a variety of professional backgrounds. In this process, psychoanalysis moved beyond the boundaries of a medical specialty and developed into a broader discipline concerned with human beings and culture. Yet the expansion of diversity also brings new tasks and challenges. If, in emphasizing diversity, the environment paradoxically makes it difficult for a particular profession to articulate its distinctive experience and perspective, the very foundation of diversity may itself be weakened. Non-discrimination does not mean non-differentiation. For psychiatrists, psychoanalysis is both a theory for understanding the unconscious and human desire and a clinical tool for understanding and treating people suffering from mental disorders. Psychiatrists are responsible for psychopathology and diagnosis, the differential diagnosis of physical and mental illness, pharmacotherapy, the assessment and management of suicide risk and emergencies, and the selection and integration of different treatment modalities. Sabshin (1985) regarded precisely these issues of diagnosis and clinical triage as central to the relationship between psychiatry and psychoanalysis. Quite apart from technical proficiency in psychoanalysis, deciding which patients are suitable for psychoanalysis, which would be better served by psychotherapy, and which require medication or other medical interventions is an important part of clinical responsibility. The particular position of psychiatrist-psychoanalysts can also be seen in the history of professional organizations. The American College of Psychoanalysts was founded in 1969 as an organization for psychoanalysts who were psychiatrists (Clemens, 2016). The College viewed the psychoanalytic practice of psychiatrists as having distinctive features derived from medical school and residency training: a diagnostic perspective, sustained attention to the relationship between mind and body, the integration of psychopharmacology and psychoanalysis, and familiarity with empirical research. At the same time, it explicitly maintained that just as psychoanalysts trained in psychology and social work have their own professional organizations, a distinctive community for psychiatrist-psychoanalysts is compatible with the diversity of psychoanalysis as a whole. This provides a concrete example of how preserving professional distinctiveness need not entail excluding or ranking other professions. The interdisciplinary development of psychoanalysis can therefore coexist with the development of psychoanalysis within the distinctive clinical and scholarly traditions of individual professions. Psychiatrists should be able to integrate two professional identities—psychiatrist and psychoanalyst—without relinquishing their medical identity. Such integration should not be left solely to the individual. If it is to be sustained within education and institutions, psychiatrists need not only a scholarly organization in which they can share and study their clinical experience, but also an independent training system that incorporates their distinctive professional expertise into the formation of psychoanalysts. A Crisis of Psychoanalytic Identity among Psychiatrists The demedicalization of American psychoanalysis and the decline in physician applicants illustrate this issue symbolically. According to Cooper (1990), over roughly a decade from 1978–1979 to 1987–1988, the total number of applicants for psychoanalytic training in the United States declined by 30%; during the same period, the number of non-physician applicants doubled, while physician applicants declined by approximately 50%. These figures should not be interpreted simply as a consequence of non-discrimination policies, but Cooper understood them in relation to the broader process of the “demedicalization” of psychoanalysis. He was concerned that as psychoanalysis ceased to be perceived as a medical specialty, young psychiatrists who had completed many years of medical education and psychiatric residency would find it increasingly difficult to choose a psychoanalytic training path that seemed discontinuous with their prior medical education. Cooper’s (1990) concern extended beyond the declining number of psychiatrists. He anticipated that as psychiatrist-psychoanalysts became fewer, psychoanalysis could become increasingly distant from departments of psychiatry in medical schools, residency education, and clinical research, thereby weakening its clinical and scientific foundations. Psychoanalytic research, in particular, requires connections with the research infrastructure, clinical resources, and scientific training available in universities and medical schools; separation from psychiatry could therefore affect the research capacity of psychoanalysis. Kernberg (2006) also strongly criticized the growing distance between psychoanalytic institutes and psychiatry. He argued that this separation limited dialogue with emerging research on affect and biological psychiatry and that such intellectual isolation impeded the creativity and scientific development of psychoanalysis. The growing distance between psychiatrists and psychoanalysis is also related to changes within psychiatry itself. Modern psychiatry has developed rapidly around biological medicine, psychopharmacology, neuroscience, and evidence-based medicine. These developments have produced important achievements in the treatment and study of mental disorders, but they have also had the unintended effect of relatively weakening traditions that explore patients’ subjective experience, unconscious meaning, and relational context in depth. Sabshin (1998) expressed concern about a decline in “psychological-mindedness” among American psychiatrists. By psychological-mindedness, he meant the capacity to identify meaningful and causal connections among psychological processes such as childhood experience, major affects, defenses, personality structure, and transference. He emphasized that this capacity is not opposed to biological psychiatry but complements it, and that psychoanalysts should remain actively involved in medical-school and psychiatric-residency education. This remains an ongoing issue. Agrawal (2024) reports that psychiatrists’ interest in psychoanalysis has steadily declined over recent decades and that APsA, recognizing this as a new challenge, established the Michael Balint Scholarship in 2023 to encourage medical students’ interest in psychodynamic and psychoanalytic thinking. The program received 25 applications in its first year and 31 in its second, suggesting that medical students’ interest in psychoanalysis has not disappeared. Rather, it points to the need for pathways and professional models through which medical students can encounter psychoanalysis and develop that interest further. The Need for Psychoanalytic Training Designed for Psychiatrists Arguing for an independent psychoanalytic training pathway for psychiatrists does not mean that psychoanalytic education as a whole should be divided into closed tracks according to profession. The central point is that training for psychiatrists should not stop at acquiring general psychoanalytic theory and technique. It should be designed to integrate psychoanalytic thinking with their pre-existing expertise in psychiatric diagnosis and psychopathology, pharmacotherapy, the relationship between mind and body, risk assessment, and clinical decision-making. Kernberg’s critique of psychoanalytic education offers important implications in this regard. Kernberg (1986) warned that psychoanalytic education could come to resemble dogmatic transmission rather than open scientific inquiry. He argued that psychoanalytic institutes should function not as “seminaries” or closed technical schools but as university-like institutions that transmit and investigate knowledge and generate new knowledge. In his model, psychoanalytic education should not merely reproduce the authority of a particular school; it should critically examine competing theories and techniques, incorporate current clinical and scientific knowledge, and develop both therapeutic skill and an investigative attitude. From this perspective, a psychoanalytic community composed of psychiatrists should likewise be more than an organization protecting professional interests: it should be a scholarly community that critically studies and develops psychoanalysis on the professional foundation of psychiatry. Kernberg (2006) again emphasized that authoritarianism, excessive hierarchy, neglect of research, and isolation from outside disciplines within psychoanalytic education can inhibit trainees’ creativity and professional development. In particular, he argued that excluding interests such as psychoanalytic psychotherapy, group therapy, couples therapy, and neuroscience in the name of protecting psychoanalytic identity may not preserve that identity at all, but instead reflect a fear-based conservatism. This has particular significance for psychiatrist-psychoanalysts. Becoming a psychoanalyst does not mean that a psychiatrist relinquishes responsibility for psychiatric diagnosis, pharmacotherapy, neuroscience, medical ethics, or emergencies. Psychoanalytic training should add a deep understanding of the unconscious, transference, countertransference, defenses, and relational meaning to existing medical expertise and integrate them into a coherent clinical identity. To sustain this goal, a training system is needed that includes training analysts, supervising analysts, a curriculum, and a peer group who share an understanding of the clinical realities and responsibilities of psychiatrists. Psychoanalysis as a Clinical Treatment and Modern Psychiatry Before psychoanalysis became a theory of human beings and culture, it began in the clinical encounter. Freud’s discoveries emerged from attempts to understand and treat the symptoms of patients with hysteria. Psychoanalysis later developed into a broad theory of the human mind, but therapeutic practice remains one of its central foundations. Thus, even as contemporary psychoanalysis expands into diverse academic and cultural domains, it cannot abandon the question of how to understand and treat patients suffering from mental disorders. Contemporary psychiatric practice, in particular, encounters a much broader range of patients than the neurotic patients who were the principal focus of classical psychoanalysis. Severe personality pathology, self-harm and suicide risk, bipolar disorder and psychotic states, developmental disorders, and complex interactions between physical and mental illness are part of everyday psychiatric practice. In treating such patients, psychiatrists must combine psychodynamic understanding with diagnostic judgment, risk assessment, pharmacotherapy, modification of the treatment setting, and realistic treatment planning. Sabshin (1998) regarded psychiatrists’ ability to maintain competence in psychotherapy as a central factor in determining the future identity not only of psychoanalysis but also of psychiatry. He warned that if psychiatrists devalue and neglect psychological-mindedness and analytic skills, psychiatry may lose its intellectual leadership in understanding subjective human experience and become reduced to the role of a “medication prescriber.” This would mean that psychiatry had relinquished its own distinctive professional identity as a field capable of integrating biological treatment and psychological understanding in the care of the whole person. In such circumstances, psychoanalysis itself could also be fundamentally destabilized. If it lost the leadership and influence of psychiatrists who integrate biological treatment with psychotherapy, the “medical rigor” of psychoanalysis could be diluted, making it more difficult to sustain its identity as a science. The perspective, skills, and clinical experience through which psychiatrists integrate pharmacotherapy and psychotherapy are therefore important features that distinguish them from non-physician analysts. Psychiatrist-psychoanalysts have an important contribution to make to the clinical and theoretical research needed in this area, and a system is required in which they can exercise their distinctive clinical leadership. Kernberg (2007) argued that psychoanalytic education should actively incorporate these realities of contemporary clinical practice. He criticized the tradition of excluding psychoanalytic psychotherapy from psychoanalytic training or treating it as secondary, arguing that such an approach fails to develop the therapeutic competencies required for many of the patients encountered in actual practice. Teaching the indications and contraindications, similarities and differences, and techniques of both standard psychoanalysis and psychoanalytic psychotherapy at a high level, he argued, can clarify the specificity of psychoanalytic technique and enable clinicians to apply psychoanalytic knowledge to a wider range of patients. Kernberg (2007) further emphasized that psychoanalytic institutes should restore their connections with universities, particularly departments of psychiatry and psychology, and actively engage with developments in adjacent fields such as neuroscience and psychopharmacology, as well as with normal and pathological mental development, psychopathology, and treatment technique. Research is central to this connection. Rather than remaining isolated within clinical experience and interpretive traditions, psychoanalysis should develop a broad range of clinical, naturalistic, historical-hermeneutic, and empirical research and collaborate with other disciplines in critically examining its own hypotheses. From this perspective, a psychoanalytic scholarly organization for psychiatrists and a psychoanalytic training system for psychiatrists cannot be separated. The scholarly organization provides an intellectual foundation linking psychoanalysis with contemporary psychiatry, psychopharmacology, neuroscience, and psychotherapy research; the training system ensures that this knowledge and clinical attitude are actually transmitted to the next generation of psychiatrist-psychoanalysts. When the two institutions work together, psychiatrists can retain psychoanalytic depth while incorporating developments in modern psychiatry, and psychoanalysis can remain connected to the realities of psychopathology and clinical treatment. Why Psychiatrists Need Their Own Psychoanalytic Scholarly Organization and Training System We can now return to the central question of this paper: Why is there a need for a psychoanalytic scholarly organization composed of psychiatrists and a psychoanalytic training system designed for psychiatrists? First, such institutions are needed to sustain the integration of psychiatrists’ medical responsibilities with psychoanalytic thinking. Psychiatrists must understand patients’ unconscious processes and transference while also evaluating psychopathology, physical illness, pharmacotherapy, suicide risk, and emergencies and choosing among different treatment modalities. Studying and teaching the meaning of these complex responsibilities within psychoanalytic treatment is a distinctive task for psychiatrist-psychoanalysts. An independent scholarly organization can provide a continuing forum for this inquiry, while a psychoanalytic training system can incorporate it directly into the formation of trainees’ clinical identity. Second, they are needed to preserve a psychiatric perspective within psychoanalysis. As Cooper (1990) and Sabshin (1985) warned, if psychoanalysis and psychiatry become separated, psychoanalysis risks gradually losing contact with severe psychopathology, diagnosis, pharmacotherapy, and the medical system. Conversely, psychiatry may lose the depth that comes from attending to the unconscious and the meaning of relationships. A scholarly organization and training system for psychiatrists are not intended to assert the superiority of one field over another; rather, they can serve as institutional means of maintaining the interface between the two fields and reducing the losses that separation may bring. Third, they are needed to preserve the tradition of psychoanalytic thinking and psychotherapy within psychiatry and transmit it to the next generation. As discussed above, the weakening of what Sabshin (1998) called “psychological-mindedness” may represent not only a crisis for psychoanalysis but also an erosion of psychiatry’s clinical depth. Maintaining the capacity to understand patients not simply in terms of diagnoses and medication response but in the context of developmental history, relationships, defenses, desires, and conflicts requires the continuing participation of psychiatrist-psychoanalysts in psychiatric education. This cannot depend solely on the efforts of individual analysts; it requires a stable training base capable of producing future educators and supervisors. Fourth, independent organizations and training systems of this kind need not contradict the interdisciplinarity of psychoanalysis; they may instead represent one way of realizing it. The example described by Clemens (2016) shows that analysts from different professional backgrounds—psychology, social work, psychiatry, and others—can maintain their own professional communities while sharing a common foundation within the broader psychoanalytic field. A unified psychoanalytic community and profession-specific communities need not be competitors. What matters is that each community develop its distinctive expertise and remain in active exchange with others without excluding or ranking other professions. Finally, these institutions are needed to make psychoanalysis once again a realistic and professionally coherent career path for psychiatrists. The Michael Balint Scholarship described by Agrawal (2024) is a recent effort to give medical students concrete opportunities to encounter psychodynamic and psychoanalytic thinking. Yet if such interest is to develop beyond a one-time experience into psychiatric residency education and eventually psychoanalytic formation, medical students and psychiatrists need a viable professional model demonstrating that one can become a psychoanalyst without relinquishing one’s medical identity. A scholarly organization and training system for psychiatrists can provide the institutional foundation for making such a pathway visible and sustainable. Conclusion: Diversity and Professional Distinctiveness Are Not Opposed The history of psychoanalysis has been a history of expansion. Although psychoanalysis began within medicine, its encounters with psychology, the humanities, art, and the social sciences have transformed it into a far richer intellectual tradition. The broad opening of psychoanalytic education and membership is an important part of this historical development. Protecting diversity, however, does not require erasing the differences and distinctiveness that arise from different professional backgrounds. Non-discrimination should not be confused with non-differentiation. Recognizing people from different professional traditions as equal psychoanalysts is fully compatible with recognizing the different knowledge, responsibilities, and clinical experiences they bring to psychoanalysis. A mature interdisciplinary community should enable its members to develop their own professional identities while meeting and engaging with one another. In this sense, the existence of a psychoanalytic scholarly organization composed of psychiatrists and a psychoanalytic training system designed for psychiatrists is not contrary to the diversity of psychoanalysis. Rather, it is a way of preserving and developing medical and psychiatric psychoanalysis as one professional tradition within the broader psychoanalytic community. Its purpose is to narrow the current gap in which psychiatrists are becoming more distant from psychoanalysis and psychoanalysis more distant from psychiatry, and to renew the longstanding dialogue between the two fields under contemporary clinical and scientific conditions. For psychiatrists, psychoanalysis is not merely a legacy of an earlier era in psychiatry. It is a mode of clinical thinking that enables the clinician to understand the patient as a subject rather than as a collection of symptoms and diagnoses, and an important intellectual and clinical tradition that makes it possible to integrate biological and psychological treatment within the care of one person. At the same time, within psychoanalysis, psychiatrists can provide an important bridge to the worlds of psychopathology, medical diagnosis and treatment, neuroscience, and research. Now that psychoanalysis has opened its doors to a wide range of professions, the question is whether the institutional space exists for each of the professional traditions that constitute this diversity to develop its own distinctiveness fully. For psychiatrists, that space needs to extend beyond an informal community and take the form of an independent scholarly organization and a psychoanalytic training system. A psychoanalytic scholarly organization and training system for psychiatrists can provide one practical answer to this question. If such institutions maintain an open and critical scholarly attitude, sustained engagement with contemporary psychiatry and neuroscience, broad clinical competence including psychoanalytic psychotherapy, and active exchange with psychoanalysts from other professional backgrounds, they do not represent a return to the exclusionary medicalism of the past. Rather, they can provide a foundation for integrating psychiatrists’ medical expertise with psychoanalytic identity, transmitting the dialogue between psychiatry and psychoanalysis to the next generation, and making the interdisciplinary diversity of psychoanalysis more substantive and more robust.   • The Survival of the Disabled : A Psychoanalytic Interrogation of Ableism[1]

    손홍석 2026-09-04 The eight survivors at Marronnier Park[2] “A life of freedom! Out of the institution!” shouted the eight men in wheelchairs at Marronnier Park. It was the 4th of June 2009, the beginning of a long, sixty-two-day outdoor sit-in demanding deinstitutionalization and the establishment of a fair policy for the independent living of disabled people. The eight men, all of them severely physically impaired, were residents of a segregated institution and came to be known as ‘the Marronnier Eight’. They had started their protest against the residential facility with accusations of human rights violations. The monumental resistance at Marronnier Park stirred up a powerful dynamic in Korea’s deinstitutionalization movement, which resulted in the establishment of new policies and substantial changes. After more than twenty years of life in the institution, they threw a stone at the glass wall of an ableist society. They were fighting not only for their own independent lives but also for an inclusive community. They were the survivors. The illusions of ableism Ableism is “a network of beliefs, processes and practices that produces a particular kind of self and body that is projected as the perfect, species-typical and therefore essential and fully human” (Campbell, 2001). It is a system that installs able-bodiedness as the norm. This criterion makes people view disability as a deficit measured against the standard. It activates the mechanism of representation and assimilation to reduce differences to sameness, thereby othering disability. Beneath this system stand the creeds of the modern Enlightenment, which hold reason, autonomy, and independence to be human ideals. Those ideals urge subjects to achieve complete control of themselves; correspondingly, losing autonomy becomes something to be feared and avoided. An ableist society regards those who must depend on others as worthless. People who think of themselves as normal see disabled people only from a solipsistic standpoint, projecting onto them their own fantasy of omnipotent control. Through this viewpoint, people who cannot fulfill the universal criteria are seen as objects, as something less than human. Disabled people are objectified: as those to be cared for, pitied, despised, or, at worst, disposed of. They cannot be recognized in their own right; they enter awareness only once they have been represented by, or assimilated into, the ableist society. When projected onto the able-bodied self, the same fantasy gives rise to the compulsion towards a perfect body. However, this is only a feigned serenity supported by the denial of interdependence. This illusory aspect shows itself in the perplexity, or the faint fear, that arises when we encounter “extraordinary bodies” (Garland-Thomson, 1997), bodies whose strangeness and ambiguity cannot easily be incorporated into the confined ego of able-bodiedness. The disabled body discloses the fundamental vulnerability of all, shattering the fantasy of controlling our bodies perfectly. The overlooked but inevitable fact that we are only temporarily able-bodied[3] reminds us that we were all once dependent on somebody from the beginning and will be so at some point before the end of our lives. Scuro asks a penetrating question of the ableist who could never imagine putting himself or herself in another’s place, or even in the place he or she began from: The violence permissible with ableist ideologies emerges as a form of thoughtlessness because no one has thought to ask: who is really the narcissist here? Is it the person who has come to identify with a disability or is it the ablebodied person who designates then denigrates the other (but “not me”; “never me”) as disabled? (Scuro, 2018) The survival of the object and the destruction of fantasy The illusions of ableism—the fantasy of omnipotent control and the negation of interdependence­—produce a characteristic relational pattern: otherness becomes unthinkable and the disabled body is excluded. This pattern is comparable to what Winnicott called “object-relating” (Winnicott, 1969). He coined the term to describe a relationship in which the infant treats the mother not as a separate entity but as a continuum of the self. Here the m/other is only a “subjective object”, a phenomenon created by the infant’s omnipotent projections—the baby has created the breast (Winnicott, 1969). The mother has been there all along as a facilitating environment, but her subjectivity is not yet recognized: the baby’s needs are met without any awareness that she is the one meeting them. “Object-usage”, by contrast, is the “acceptance of the object’s independent existence” (Winnicott, 1969), and the passage from the one to the other is, for Winnicott, a crucial developmental achievement—the acquisition of a capacity to recognize the m/other as a real object external to the self. According to Winnicott, a development from “object-relating” to “object-usage” necessarily depends on the “survival of the object” (Winnicott, 1969). The mother has to survive the infant’s destruction of her, which manifests itself as endless demand and ruthless attack. If the mother survives,[4] the infant begins to move away from relating subjectively. The mother is then perceived as “the object as an external phenomenon, not as a projective entity, in fact recognition of it as an entity in its own right” (Winnicott, 1969). Having developed the capacity to use an object—that is, to relate with an “object outside the area of the subject’s omnipotent control” (Winnicott, 1969)—the baby can now live in a world of shared reality with others. Recognizing otherness in its own right is a premise of authentic love, of concern for the other, and of a mature subjectivity of the self (Winnicott, 1962). The survival of the disabled makes it possible to destroy the omnipotent fantasy of ableism. And destroying that fantasy is essential if we are to move from conceiving otherness as a deficit to embracing it as a difference per se. Neither the ableist’s tolerance of disability nor the assimilation of disabled people into ableist society brings about a fundamental change. It is the survival of the disabled that deconstructs the ableist society. The appearance of the Marronnier Eight, and what followed from it, has shown us this simple truth. The fear of WOMAN and acknowledgement of interdependence The negation of dependence is the second pillar of ableism, and it accounts for much of society’s cruel prejudice and brutal violence towards disability. As Benjamin puts it, “the self finds it intolerable to bear the vulnerability of being dependent on another subject whom he does not control, indeed who is independent and can demand the same recognition as the Self” (Benjamin, 2018). To disregard one’s dependence on others may lead to fear of the object one depends upon. In a similar vein, Winnicott identified the “fear of WOMAN” as the root of misogyny (Abram, 2007). He insisted on both its palpability and its universality: This fear of WOMAN is a powerful agent in society structure, and it is responsible for the fact that in very few societies does a woman hold the political reins. It is also responsible for the immense amount of cruelty to women, which can be found in customs that are accepted by almost all civilizations. The root of this fear of WOMAN is known. It is related to the fact that in the early history of every individual who develops well, and who is sane, and who has been able to find himself, there is a debt to a woman—the woman who was devoted to that individual as an infant, and whose devotion was absolutely essential for that individual’s healthy development. The original dependence is not remembered, and therefore the debt is not acknowledged, except in so far as the fear of WOMAN represents the first stage of this acknowledgement. (Winnicott, 1950)[5] Winnicott urges us to recognize the universal fact that every individual once depended on someone else at the initial stage of life; recognizing it would lessen the fear inside ourselves. This he takes to be a prerequisite for a society’s progress (Winnicott, 1957). Equating the fear of WOMAN with the fear of dependency, he traces to it the source of the cruelty directed at the object the self once depended upon. I would add that this cruelty can also be turned toward the ones who expose the veiled fact of dependency, the disabled. Acknowledging our interdependence as a fundamental human condition will therefore pave the way for deconstructing ableism. This is not a difficult truth to be acquired but a plain fact we have forgotten, concealed by fear and defended by the fantasy of complete independence and autonomy. Care from others is indispensable if disabled people are to survive—and it is no less indispensable for everyone else. Care, then, should not be a charity offered in one direction, committing the unwitting violence of endeavouring to normalize the other, but a reciprocally shared way of living in solidarity.   • 정동에 대한 신경정신분석적 이해의 활용

(생략) 정동은 정신분석에서 중요한 위치를 차지한다. 환자들은 불안, 우울, 분노와 같은 정동이 주는 괴로움을 해결하고자 분석을 찾기에 정동은 치료의 계기이자 목표가 된다. 치료자는 환자가 치료실 안팎에서 나타내는 정동을 바탕으로 환자의 마음을 이해한다. 더 나아가 치료자는 정동을 환자의 변화를 이끌어내는 치료의 수단으로 사용하기도 한다. 그러나 정동은 때때로 그 의미와 원인을 파악하는 것이 어렵다. 정동에는 여러 요소가 영향을 미치기 때문이다. 비교적 쉽게 확인할 수 있는 현실 속에서의 사건 뿐 아니라 방어기제, 무의식적 소망 등이 뒤얽혀 정동을 형성한다. 따라서 정동을 어떻게 바라볼 것인지에 대해 많은 이론들이 서로 입장을 달리 하는 것은 인간의 정동이 가지는 복잡성에서 기인한 필연적인 결과라고 할 수 있다. 환자가 치료자에게 분노할 때 자기심리학에 기반을 둔 치료자는 공감 실패에 따른 자연스러운 현상으로 이를 바라보고 다룸으로써 환자가 공고하게 자기를 형성하도록 도우려 할 수 있다. 반면 자아심리학 기반의 치료자는 환자 분노의 원인이 된 소망과 전이에 초점을 맞추고 해석하려 할 것이다. 정동을 다루는 수 많은 이론과 이해의 틀에도 불구하고 임상 현장에서 환자를 이해하는 데에는 여전히 어려움이 있다. 이 글에서는 정동에 대한 신경심리학적 시각이 환자를 이해하고 치료하는 데에 어떻게 도움이 될 수 있을지, PANIC이라는 감정에 초점을 맞추어 살펴보려 한다. 판크세프는 포유류 뇌에 SEEKING, RAGE, FEAR, LUST, CARE, PANIC/GRIEF, PLAY라는 7개의 일차 정동 체계가 존재한다고 제안했으며, 이 중 PANIC/GRIEF(초기에는 PANIC으로만 명명)는 중심회백질(PAG)에서 전측대상피질로 이어지는 회로에 자리하여 분리 시 울음소리(protest call)를 매개한다고 보았다. 이 시스템은 오피오이드·옥시토신·프로락틴에 의해 억제되고 CRF·글루타메이트에 의해 활성화되며, 사회적 접촉(특히 신체 접촉)에 의해 하향조절된다. 중요한 점은 판크세프가 이 PANIC/GRIEF 체계를 FEAR 체계와 해부학적·자율신경계적으로 명확히 구분했다는 것이다. (생략) 정신분석에 신경심리학적 지식을 통합하려는 시도에 대해서는 많은 비판이 있다. 정신분석 학계의 일부에서는 이러한 시도가 인간의 복잡한 마음을 신경심리에서 주로 관심을 기울이는 인지적인 측면으로 환원해버린다고 비판한다. 우리가 PANIC이라는 감정을 모두 동일한 기전에서 오는 동일한 정동이라고 치부한다면 그 비판 그대로 개인의 복잡한 내면을 이해하려는 정신분석의 기본 정신을 잃는 일이 되어버릴 수도 있다. 그러나 우리가 과학적 기반을 가진 신경정신분석이라는 이론을 환자의 마음을 이해하는 (유일하지 않은!) 도구 중 하나로 활용한다면 그것은 다른 정신분석적 이론들을 활용하는 것과 마찬가지로 무한히 복잡한 사람의 마음을 이해하는 시각을 넓히는 일이 될 것이다. Patients seek analysis in an effort to alleviate the suffering associated with affects such as anxiety, depression, and anger; affect thus constitutes both an impetus for treatment and a therapeutic goal. Analysts seek to understand patients’ minds on the basis of affects expressed both within and outside the consulting room. Furthermore, affect may itself be used as a therapeutic medium through which change can be facilitated. At times, however, it is difficult to determine the meaning and origins of affect because it is shaped by multiple factors. Affect is formed not only through events in external reality that may be relatively easily identified, but also through the complex interplay of defense mechanisms, unconscious wishes, and other intrapsychic processes. Accordingly, the existence of divergent theoretical positions regarding how affect should be understood may be seen as an inevitable consequence of the complexity inherent in human affect. When a patient becomes angry with the analyst, a therapist working on a self-psychological perspective may understand and address this as a natural response to empathic failure, thereby helping the patient establish a more cohesive self. In contrast, a therapist working on a ego psychological perspective may focus on and interpret the wishes and transference underlying the patient’s anger. Despite many theories and conceptual frameworks available for understanding affect, clinicians continue to encounter difficulties in comprehending patients in everyday practice. This paper examines how a neuropsychoanalytic perspective on affect may contribute to understanding and treating patients, with particular attention to the affective system of PANIC. Panksepp proposed that the mammalian brain contains seven primary affective systems: SEEKING, RAGE, FEAR, LUST, CARE, PANIC/GRIEF, and PLAY. He located the PANIC/GRIEF system—initially designated simply as PANIC—in a circuit extending from the periaqueductal gray (PAG) to the anterior cingulate cortex, where it mediates separation-induced distress vocalizations, or protest calls. This system is inhibited by opioids, oxytocin, and prolactin, activated by corticotropin-releasing factor (CRF) and glutamate, and downregulated by social contact, particularly physical contact. Importantly, Panksepp clearly differentiated the PANIC/GRIEF system from the FEAR system at both anatomical and autonomic levels. Whereas FEAR is characterized by sympathetic activation, including increased heart rate and sweating, PANIC/GRIEF is characterized by parasympathetic predominance, including chest constriction, a lump-in-the-throat sensation, and an urge to cry. Francesetti proposed that panic attacks should be reconceptualized not simply as “excessive fear responses,” but as “acute attacks of solitude” arising when an individual is separated from social and emotional support and becomes excessively exposed to the world. From this perspective, fear is not the primary phenomenon in panic attacks; rather, it emerges secondarily following the more primordial experience of feeling that one may die or go mad. Agoraphobic avoidance may consequently be understood as avoidance of re-separation—that is, of being alone again. However, if neuropsychoanalysis, as a scientifically grounded theoretical framework, is used as one—though certainly not the only—tool for understanding the patient’s mind, it may broaden our perspective on the infinitely complex human psyche, just as other psychoanalytic theories do.

동시에 타자가 자신보다 강해지거나 독립적인 존재로 경험되는 것은 견딜 수 없다. 따라서 타자는 살아 있어야 하지만, 결코 주체를 위협해서는 안 된다. 이 긴장은 전능적 흥분이라는 형태로 유지된다. 주체는 타자를 지배하고 반응하게 만들면서, 그 반응을 통해 자신의 존재감을 확인한다. 중요한 점은, 이 구조가 단순한 전능감이나 자기과대적 환상이 아니라는 것이다. 전능적 흥분은 차라리 자기 붕괴를 막기 위한 정교한 조절 장치이며, 공격성이 자기 자신을 파괴하지 않도록 방향을 바꾼 결과이다. 다시 말해, 이는 자기애적 주체가 감당할 수 있는 범위 내에서 공격성을 ‘살려두는’ 방식이다. 자기애적 주체는 이 구조를 통해 안전한 자기감을 유지한다. 그러나 그 대가로 관계는 상호성을 잃고, 타자는 주체의 내적 균형을 유지하기 위한 기능적 존재로 전락하고 만다. 4. 전능적 흥분과 관계의 왜곡: 타자의 기능화 전능적 흥분이 조직화되는 순간, 타자는 더 이상 독립된 욕망과 내적 세계를 지닌 존재로 경험되지 않으며, 주체의 정서 조절과 자기 영속성을 유지하는 도구적 대상(object)으로 기능한다. 이러한 기능화는 단순한 대상화(objectification)와는 구별된다. 대상화가 타자를 단순한 물건이나 수단으로 환원하는 것이라면, 기능화는 타자를 ‘살아 있는 채로’ 유지하면서도 그 살아 있음이 오직 주체의 안정성을 보조하는 범위 내에서만 허용되는 상태를 의미한다. 다시 말해, 타자는 살아 있으되 스스로의 삶을 갖지 못하며, 주체의 심리적 평형을 유지하는 장치로서만 존재한다. 이 구조 속에서 타자의 반응은 필수적이다. 그러나 그것은 상호적인 반응이 아니라, 주체의 전능성을 반사하고 확인시켜 주기 위해 필요한 반응이다. 타자는 주체의 욕망에 반응함으로써 주체가 여전히 영향력을 지니고 있음을 증명해 주는 역할을 하며, 그 반응의 강도와 형태는 주체의 불안을 조절하는 기능을 수행한다. 중요한 점은, 이러한 관계 왜곡이 단순한 권력욕이나 지배욕의 산물이 아니라는 점이다. 이는 자기애적 주체가 경험하는 존재론적 불안을 관리하기 위한 생존적 적응이다. 타자를 완전히 제거하면 자기 소멸의 공포가 증폭되고, 반대로 타자가 완전히 자율적 존재로 다가오면 동일한 공포가 다시 활성화된다. 따라서 타자는 “살아 있으나 위협적이지 않은 상태”로 유지되어야 하며, 이것이 바로 기능화된 관계의 핵심이다. 이와 같은 관계 구조에서는 진정한 상호성이 발생하기 어렵다. 타자는 관계의 주체라기보다, 주체의 내적 균형을 지탱하는 장치로만 작동하기 때문이다. 그 결과 관계는 지속되지만 변화하지 않으며, 상호적 성장보다는 안정적 반복만이 강조된다. 이는 주체가 타자와 함께 변화하는 대신, 변화를 차단함으로써 자기 동일성을 보존하려는 시도라 할 수 있다. 타자는 주체를 변화시키지 못하며, 주체 또한 타자에 의해 변형될 수 없다. 따라서 관계는 유지되지만, 상호 주체성은 발생하지 않는다. 이러한 상태는 안정적이지만 정체된 관계이며, 바로 이 지점에서 자기애적 병리는 주체와 객체간의 진정한 상호작용을 봉쇄하는 형태로 고착된다. 5. 자기대상화(selfobjectification)와 관계의 내면화 전능적 흥분이 관계의 외적 구조를 변형시키는 방식이라면, 자기대상화(selfobjectification)는 그 구조가 주체의 내면에 고착되는 양상이라 할 수 있다. 즉, 전능적 흥분이 타자를 기능화하는 외적 관계의 조직 원리라면, 자기대상화는 그러한 관계 양식이 주체의 내적 세계 안으로 내면화되는 과정이다. 앞서 논의했듯이, 전능적 흥분 속에서 타자는 살아 있으되 주체의 안정과 연속성을 보장하는 기능적 존재로 경험된다. 이러한 관계가 반복될수록, 주체는 점차 외부 대상과의 실제적 상호작용보다, 자신의 내적 표상 속에서 구성된 대상과의 관계를 더 안전한 관계 양식으로 내면화하게 된다. 이때 형성되는 관계는 진정한 의미의 상호주체적 관계라기보다, 자기 안정화를 위해 조직된 자기대상적 관계에 가깝다. 그러나 여기서 말하는 자기대상(selfobject)은 코헛(Kohut)이 말한 발달적 자기대상과는 다르다. 여기에서 말하는 자기대상화는, 아직 타자의 자율성이 충분히 승인되지 않은 상태에서 형성된 관계 구조를 가리킨다. 다시 말해, 타자는 외부에 실재하지만 주체의 경험 속에서는 독립된 타자가 아니라 자기 조절을 위해 기능하는 심리적 장치로 작동한다. 이러한 자기대상화는 주체에게 중요한 심리적 이점을 제공한다. 외부 세계의 예측 불가능성과 관계적 위험으로부터 벗어나, 관계의 불확실성을 내부에서 통제할 수 있게 된다. 그러나 이 안정성은 대가를 동반한다. 자기대상화된 관계에서는 상호 변형의 가능성을 상실한다. 타자는 주체를 변화시키는 존재가 아니라, 주체의 항상성을 유지하는 장치로 기능한다. 이때 관계는 유지되지만, 관계를 통해 새로운 의미가 생성되지는 않는다. 또한 이러한 상태는 주체로 하여금 외부 관계에 대한 욕구를 제한된 방식으로만 경험하게 한다. 관계는 필요하지만 위험하며, 친밀함은 원하지만 그것이 가져올 변화는 견딜 수 없다. 따라서 주체는 타자를 욕망하면서도 동시에 통제하려는 이중적 태도를 취하게 된다. 이 긴장은 전능적 흥분을 통해 조절되며, 주체는 관계를 완전히 상실하지 않으면서도 관계가 자신을 변화시키지 못하도록 방어한다. 6. 전능적 흥분의 해체와 관계의 재구성 전능적 흥분은 자기애적 주체가 자기 소멸의 위협 속에서 살아남기 위해 형성한 방어적 조직이다. 따라서 전능적 흥분의 해체는 그 자체로 또 하나의 위기를 동반하며, 성급한 해체는 오히려 주체의 붕괴를 초래할 수 있다. 전능적 흥분이 완화되기 위해서는, 그것이 더 이상 필요하지 않다는 경험이 반복적으로 축적되어야 한다. 이는 해석이나 통찰 이전에, 관계적 안전(relational safety)이 충분히 확보되는 과정을 필요로 한다. 다시 말해, 주체가 자신의 공격성과 의존성이 드러나더라도 관계가 파괴되지 않는다는 경험이 반복적으로 제공될 때에만, 전능적 흥분은 점차 그 필요성을 잃는다. 이 과정에서 핵심적인 전환점은 ‘대상의 생존(survival of the object)’에 대한 실제적 경험이다. 위니캇이 말한 것처럼, 대상은 공격을 견디고도 붕괴되지 않으며, 동시에 복수나 철회로 대응하지 않는다. 이러한 경험은 주체로 하여금 처음으로 다음과 같은 인식을 가능하게 한다.“내가 파괴적일 수 있음에도 불구하고, 관계는 무너지지 않는다.” 전능적 흥분이 약화되기 시작하면, 주체는 더 이상 타자를 통제함으로써 자신을 지키지 않아도 된다는 가능성을 감지하게 된다. 이때 관계는 기능적 장치에서 점차 상호적 공간으로 이동한다. 타자는 더 이상 주체의 안정성을 보증하는 도구가 아니라, 예측 불가능하지만 살아 있는 존재로 경험되기 시작한다. 이 전환은 결코 일방향으로만 이루어지지 않는다. 오히려 불안정하고 반복적인 후퇴를 동반한다. 전능적 흥분은 완전히 사라지기보다는, 관계가 위협적으로 느껴질 때마다 재등장한다. 그러나 중요한 변화는, 주체가 이 반복을 “붕괴”가 아니라 “과정”으로 인식하기 시작한다는 점이다. 즉, 붕괴는 더 이상 절대적인 종말이 아니라, 다시 관계로 돌아올 수 있는 하나의 국면이 된다. 이러한 변화는 자기대상화의 양상에도 변화를 가져온다. 이전에는 타자가 주체의 안정성을 유지하기 위한 기능적 장치로만 존재했다면, 이제 타자는 부분적으로나마 자신의 욕망과 한계를 지닌 타자로 경험된다. 이 과정에서 핵심적인 것은, 주체가 더 이상 전능적 흥분에만 의존하지 않고도 불안을 견딜 수 있게 된다는 점이다. 공격성과 의존성, 욕망과 좌절이 동시에 존재할 수 있음을 경험하면서, 주체는 점차 자기 파괴와 관계 붕괴를 동일시하지 않게 된다. 이는 곧 우울자리의 핵심 과제—사랑과 공격성의 공존—을 부분적으로나마 감내할 수 있게 되었음을 의미한다. 결국 전능적 흥분의 해체는 관계에 대한 새로운 내적 조직이 형성되는 과정이다. 이는 자기애적 주체가 더 이상 “살아남기 위해 관계를 통제해야 하는 위치”에 머무르지 않고, “관계 안에서 흔들리면서도 살아 있을 수 있는 존재”로 이동하는 변화를 의미한다. 이러한 전환은 완결된 상태가 아니라 지속적으로 갱신되는 과정이며, 치료적 관계는 그 과정을 지탱하는 핵심이 된다. 7. 자기애의 재구성 앞서 살펴본 바와 같이, 자기애적 주체는 공격성을 단순한 충동으로 경험하지 않는다. 공격성은 대상 파괴를 넘어 자기 자신을 위협하는 힘으로 체험되며, 이로 인해 우울자리(depressive position)는 성숙의 단계라기보다 존재론적 위험 지대로 경험된다. 이러한 조건 속에서 주체는 공격성을 직접 표현하기보다는, 전능적 흥분(omnipotent excitement)이라는 형태로 전환함으로써 대상과 자기 붕괴를 회피한다. 전능적 흥분은 자기애적 병리의 핵심적인 방어이자, 동시에 자기 연속성을 유지하기 위한 욕구이다. 이는 단순한 과대자기평가나 지배 욕구가 아니라, 타자를 완전히 파괴하지 않으면서도 자신의 존재를 유지하기 위한 정교한 심리적 장치이다. 이 과정에서 타자는 제거되지 않지만 기능화되며, 관계는 유지되되 상호성은 제한된다. 즉, 주체는 타자와 관계를 맺되, 그 관계가 자신을 위협하지 않도록 통제한다. 또한 이러한 구조는 자기대상화(selfobjectification)라는 형태로 내면화되며, 주체는 점차 실제 타자와의 관계보다 자신이 구성한 관계 구조 안에서 안정성을 확보하게 된다. 이때 관계는 살아 있으나 변형되지 않으며, 타자는 주체의 자기조절을 위한 장치로 기능한다. 전능적 흥분은 관계적 안전이 확보될 때 비로소 그 필요성을 잃는다. 치료적 관계에서 대상이 반복적으로 살아남고, 주체의 공격성과 의존성을 견디며, 붕괴되지 않는 경험이 축적될 때, 주체는 점차 전능성에 의존하지 않고도 관계 속에 머무를 수 있게 된다. 주체는 더 이상 “내가 통제하지 않으면 사라진다”는 공포에 의해 관계를 유지하지 않고, 불확실성과 상호성을 감내할 수 있는 능력을 획득한다. 이 때의 자기애는 더 이상 병리가 아니라, 관계 속에서 자신을 유지하고 확장할 수 있는 역량으로 전환된다. 요컨대, 자기애의 재구성이란 전능성에 의존하지 않아도 맺어질 수 있는 관계의 발견이다. 이는 자기애의 발현을 병리로만 보지 않고, 관계를 향한 잠재적 가능성으로 다시 읽게 하는 하나의 이론적 제안이다.

  • Narcissistic Vulnerability and the Formation of the True Self: An Original Seed–Based Model of Experiential Structure

    초록 본 연구는 자기애적 병리가 개인의 의지나 성격 특성의 문제가 아니라, 초기 조율 경험 속에서 형성되는 원초적 경험 구조인 ‘Original Seed’가 특정한 생존 방식으로 경직화된 결과라는 점을 제안한다. Seed는 정동·감각·관계적 리듬이 최초로 조직되는 층위이며[24,25], 조율 실패가 반복될 때 정동 협소화, 과민한 관계 반응성, 반복적 대인 패턴과 같은 자기애적 취약성이 나타난다[28,30]. Seed의 경직화는 표상 이전 수준에서 일어나기 때문에 해석이나 인지적 통찰만으로는 변화되지 않으며[9,17], 변형은 치료 장면에서 재현되는 조율 실패의 순간을 분석가와 환자가 함께 견디는 상호주관적 경험 속에서 이루어진다[10,11,57]. 이러한 경험이 축적되면 Seed 구조는 점차 재조직되고, 존재적 방향성(Value)이 자연스럽게 발아한다[60,61]. 본 연구는 Seed–Horney–Kohut–ACT를 하나의 연속선으로 재구성하여, 자기애적 병리가 어떻게 형성되고 변화하는지를 통합적으로 설명한다. Horney는 Seed 경직화의 패턴을 지도화하고[28], Kohut은 변형이 일어나는 공감적 조건을 제공하며[30,31], ACT는 변형된 방향성이 행동으로 실현되도록 심리적 유연성을 지원한다[32,62]. 이러한 통합적 관점은 자기애적 문제의 핵심이 방어가 아니라 경험 구조의 경직화에 있으며, 변화는 새로운 관계적 조율의 내면화를 통해 발생한다는 점을 강조한다. 본문 1. 서론 임상 현장에서 관찰되는 우울, 불안, 반복적 관계 갈등, 자기애적 취약성은 표면적으로는 서로 다른 문제처럼 보이지만, 그 기저에는 개인이 세계를 경험하고 조직하는 고유한 방식, 즉 ‘경험의 형식(form of experience)’이 자리한다[1–4]. 전통적 정신역동 개념인 방어, 갈등, 전이는 이러한 경험의 형식이 이미 조직된 뒤에 나타나는 2차적 표현일 가능성이 크며[6], 정작 임상적으로 중요한 질문은 ‘왜 특정한 방식으로 세계를 경험하게 되었는가?’이다. 기존 이론들은 경험의 기원적 층위를 부분적으로 설명해 왔지만, 정동·감각·관계적 리듬이 최초에 어떻게 결합되고, 어떻게 굳어지며, 어떤 조건에서 다시 변형될 수 있는지에 대한 통합적 구조는 부족했다[9–12,18,19]. 본 연구에서 제안하는 ‘Original Seed’ 개념은 이러한 간극을 메우려는 시도로, 초기 조율 경험 속에서 형성되는 경험의 발생 구조를 하나의 임상적 단위로 재정식화하고[24,25], 이 구조가 경직화·변형·가치 출현으로 이어지는 연속적 변화를 설명한다[53,57,60]. 2. Seed의 개념적 구조 Seed는 정동, 감각, 관계적 리듬이 미분화된 상태에서 얽혀 있는 경험의 원형적 구조로, 초기 상호조율이 이 구조를 확장하거나 제한하는 발달적 기반을 형성한다[11,24]. 안정적 조율은 정동 흐름의 통합을 촉진하지만, 조율 실패의 반복은 Seed를 생존 중심의 ‘협소한 조직 원리’로 제한한다. 이는 이후 임상에서 관찰되는 패턴의 기원적 배경으로 기능한다.” Seed의 핵심 의의는, 기존 이론에서 단편적으로 논의되던 경험 발생 요소들을 임상적으로 활용 가능한 단일한 구조로 통합한다는 데 있다. Bion의 O는 경험 이전적 실재를 강조했으나[9] 임상적 변형 조건을 구체화하지 못했고, Ogden의 analytic third는 상호주관적 출현의 순간을 포착했으나[10] 그 발달적 기반은 다루지 못했다. Seed는 이러한 간극을 보완하며 경험 발생–경직화–변형의 연속적 과정을 재정식화한다[53]. 참자기와 Seed 구조: O와 관계경험의 발생적 통합 정신분석에서 ‘참자기(true self)’라는 개념은 학파에 따라 상이한 의미로 사용되어 왔다. Winnicott는 참자기를 환경의 조율 속에서 발현되는 자발성의 원천으로 보았고[4], Horney는 이를 신경증적 자기 왜곡 너머에 존재하는 성장 가능성으로 이해했으며[5,28], Kohut은 적절한 자기대상 경험에 의해 안정적으로 유지되는 건강한 자기 구조로 설명하였다[30,31]. 그러나 이들 개념은 참자기의 본질이 무엇으로 구성되는가라는 물음에 대해 충분히 통합된 답을 제시하지 못했다. 본 연구의 Seed 모델은 이러한 난점을 보완하며, 참자기를 “타고난 잠재태(O)와 초기 관계 조율 경험이 결합해 형성되는 발생적 구조”로 재정식화한다. Bion이 제시한 O는 선천적으로 주어지는 잠재적 실재이자, 상징화되기 이전의 원초적 가능성의 영역으로 이해된다[9]. 그러나 O는 그 자체로 경험이 아니며, 관계 속에서 조율될 때 비로소 감각·정동·리듬을 지닌 경험적 단위로 변환된다. Seed는 바로 이 지점에서 발생한다. 즉 Seed는 O가 초기 양육자와의 상호작용을 통과하면서 경험 가능한 형태로 조직된 최초의 구조이며, 정동적 결, 관계적 기대, 감각-리듬의 패턴이 동시에 얽혀 있는 발생적 기반이다[11,24]. 이러한 Seed의 형성 방식은 동일한 부모 아래서 자란 형제들이 매우 다른 성격 구조를 발달시키거나, 동일한 유전적 잠재태를 가진 쌍둥이가 서로 다른 양육 환경에서 전혀 다른 성향을 보이는 임상적 현실성과도 부합한다[25,52]. 이러한 관점에서 참자기란 “왜곡되지 않은 본래의 자아”라기보다, O가 관계적 조율 과정 속에서 Seed로 조직되고 발달해 나가는 생동적 경험 구조의 연속선이다. 다시 말해, 참자기는 Seed가 충분히 조율된 환경에서 발아할 때 자연스럽게 드러나는 경험적 자기이며, 특정한 정체성의 고정된 실체라기보다 정동·감각·관계적 리듬이 살아 있는 방식으로 조응하는 과정이다. 이 정의는 Winnicott적 “자발성”[4], Horney적 “성장 방향성”[28], Kohut적 “건강한 자기 구조”[30]를 모두 포괄하면서도, 이전 이론들이 충분히 설명하지 못했던 참자기의 발생 조건을 명확하게 제시한다. 결국 참자기는 타고난 잠재태(O)[9]가 초기 관계 경험을 통과하며 Seed로 조직되고 발달하는 과정을 통해 이해될 때 가장 임상적으로 설명력이 높아진다. 3. Seed 경직화와 변형의 임상 과정 Seed 경직화는 반복되는 조율 실패와 과부하·방치·과자극과 같은 초기 관계적 불균형에서 기원한다[25]. 이렇게 형성된 경직화 구조는 임상에서 정동 폭의 축소, 관계적 과민성, 반복적 상호작용 패턴, 자기애적 취약성 등으로 표현되며, 이는 보다 표면적인 방어기제 이전의 층위에서 작동한다[30]. 이러한 이유로 인지적 통찰만으로는 변화를 이끌기 어려우며, Seed가 재조직될 수 있는 경험 조건이 치료에서 핵심이 된다[17] Seed 변형은 해석이나 조언이 아니라, 치료 상황에서 재현되는 조율 실패의 장면을 분석가와 환자가 함께 견디는 상호주관적 경험 속에서 일어난다[57]. 이때 Bion의 negative capability가 구현되고[9], Ogden의 analytic third가 형성되며[10], Stern이 말한 ‘함께 살아내기(living-with)’의 경험이 발생한다[11]. 이러한 새로운 조율 경험은 점차 내면화되며 Seed의 정동·관계 구조를 재조직하고 정서적 유연성과 자기감의 연속성을 확장한다[53,58]. 4. Horney–Kohut–ACT의 통합적 위치 Seed 관점에서 Horney, Kohut, ACT는 각각 경험 변화의 서로 다른 층위를 다루며, 실제 임상에서는 이 세 이론이 시간적·발달적 순서를 이루는 구조로 통합된다. Horney의 이론은 Seed 경직화가 임상에서 어떻게 드러나는지를 가장 선명하게 포착해주는 지도 역할을 한다. 순응(moving toward), 공격(moving against), 철수(moving away)의 세 가지 경향은 단순한 성격 양식이나 대처 전략이 아니라, Seed가 반복된 조율 실패 속에서 형성된 생존 중심 조직 방식을 행동적·관계적 층위에서 드러내는 표지들이다[28]. 즉, Horney는 ‘무엇이 문제인가’를 설명하기보다 ‘경험 구조가 어떤 방식으로 왜곡되어 나타나는가’를 보여주는 기능을 한다. 특히 중요하게 재해석해야 할 지점은 Horney의 ‘통찰(insight)’ 개념이다. Horney가 말한 통찰은 인지적 명료화나 해석의 이해가 아니라, 자신의 패턴이 관계 속에서 어떻게 살아 움직이는지를 감정적으로 실감하는 과정에 가깝다[28,64]. 이는 Seed 모델이 강조하는 ‘언어 이전적 경험 구조는 해석만으로 변하지 않는다’는 명제와 충돌하지 않으며, 오히려 경직화된 Seed의 패턴을 임상 현장에서 탐색하기 위한 현상학적 지도 역할을 한다. Seed 관점에서 보면, Horney의 세 경향은 Seed 경직화의 대표적 표현 양식이며, 이 패턴들에 대한 탐색은 Seed의 구조가 어떻게 ‘생존 기반’으로 조직되어 왔는지를 이해하게 해준다. 이때 Horney의 모델은 변형을 일으키는 메커니즘 자체를 설명하는 것이 아니라, 변형이 일어나기 전에 반드시 필요한 ‘구조적 위치 파악(map-making)’을 제공한다. 즉, Horney는 Seed 변형의 전(前) 단계에서 경험 구조의 반복적 양식을 식별하고, 이후 Kohut적 공감·안정화가 그 패턴의 정동적 뿌리를 담아낼 수 있게 하는 발판을 마련한다. 이러한 관점에서 Horney는 Seed 모델 안에서 단순한 이론적 병렬이 아니라, 경직화된 경험 구조를 드러내고 탐색하기 위한 핵심적 임상 단계로 재위치된다. 다시 말해, Horney는 ‘Seed가 어떤 방식으로 굳어졌는가’를 이해하게 하고, Kohut은 ‘그 Seed가 어떤 조건에서 변형되는가’를 설명하며, 둘은 변형의 연속 과정에서 상호 보완적으로 작용한다 Kohut은 시선을 병리의 기원에서 자기애적 발달 과정 자체로 이동시키며, 적절한 자기대상 경험이 어떻게 자기 구조를 점진적으로 강화하고 복원하는지에 초점을 맞춘다[30,31]. Seed 관점에서 보면, Horney는 이미 경직화된 Seed가 어떤 패턴으로 세계를 조직하는지를 보여주고, Kohut은 Seed가 다시 발아·강화되기 위해 어떤 종류의 조율과 자기대상 경험이 반복되어야 하는지를 설명한다. ACT는 그 이후의 층위에서, 이렇게 재구성된 Seed가 만들어내는 방향성이 실제 삶의 선택과 행동으로 이어지도록 돕는 실천적 모델로 자리 잡는다[32]. 경험 회피 감소, 심리적 유연성 강화, 가치 기반 행동은 변형된 Seed가 실제 삶에서 구현되도록 돕는다[32,33,60]. 5. 자기통합 수준의 임상적 의미 자기통합 수준은 Seed와는 별개의 개념으로, 정동 조절·자기–타자 경계·자기감의 연속성과 같은 자기 기능 전반을 평가하는 임상적 지표이다[35]. Seed가 경험 발생의 기원적 구조를 설명한다면, 자기통합 수준은 현재의 기능적 안정성을 나타내며, 치료자가 어떤 개입을 어느 층위에서 우선적으로 적용할지를 결정하는 기준이 된다. 따라서 치료에서는 Seed의 구조적 경직화와 환자의 자기통합 수준을 함께 고려해 개입의 순서와 강도를 조정해야 한다.낮은 자기통합 수준에서는 원시적 방어가 우세하고 자기–타자 경계가 불안정하므로, 해석보다는 Kohut적 공감·안정화가 선행되어야 한다[30,37]. 중간 수준에서는 Seed 경직화가 반복적 관계 패턴으로 나타나며, Horney적 패턴 탐색, Kohut적 안정화, Seed 변형, ACT 실천이 자연스럽고 효과적으로 결합된다[28,29]. 높은 수준에서는 ACT가 즉시 효과를 보이며, Seed-level의 미세한 왜곡은 Horney·Kohut적 개입으로 조정할 수 있다[31]. 6. Seed-Based Protocol: 자기통합 수준에 따른 치료 순서 Seed의 경직화는 경험이 특정한 생존 중심 방식으로 조직되고 굳어지는 과정을 의미하며, 자기통합 수준은 환자가 현재 어느 정도의 정동·관계 조절 능력을 갖추고 있는지를 나타낸다. 두 지표는 개념적으로는 별개이지만 Seed의 경직화가 심해질수록 자기통합 수준이 저하되는 임상적 상관관계가 존재한다. Seed는 정동·감각·관계적 리듬이 미분화된 초발생 구조이기 때문에, 과도한 정동 자극이나 관계적 압력에 쉽게 압도되거나 파편화될 수 있다. 이러한 과자극은 Seed의 경직화 구조를 급격히 활성화시켜 공격·순응·철수 같은 생존 중심 패턴을 즉시 불러오고, 반복 enactment와 정서적 단절을 촉발하여 변형의 가능성을 오히려 차단한다. 자기통합 수준은 이러한 과자극을 견딜 수 있는 기능적 안정성을 결정하며, 자기통합이 낮을수록 Seed 과자극에 더 쉽게 취약해진다. 따라서 치료에서는 Seed가 감당할 수 있는 자극의 ‘창(Window of Tolerance)’을 유지하면서, 환자의 자기통합 수준이 허용하는 범위 안에서 새로운 경험이 구조적 변형으로 이어질 수 있도록 정동적 강도와 조율을 세밀하게 조절하는 것이 핵심이다. 낮은 자기통합 수준에서는 Seed가 쉽게 불안정해지고 파편화되기 때문에, 해석이나 패턴 탐색은 오히려 Seed를 더 취약하게 만들 수 있다[37]. 이 수준의 치료는 Kohut적 공감·안정화가 절대적으로 선행되어야 하며[30], 분석가는 자기대상적 기능을 통해 환자가 관계 속에서 안전감을 경험하도록 돕는다. 이러한 경험이 누적되어 정동적 안정이 확보되어야만, 비로소 제한적 범위에서 Horney적 패턴 자각이 가능해진다. ACT는 더욱 신중하게 적용해야 하며, 가치 탐색이 아닌 최소한의 현재 인식·회피 인식 등 안정성을 해치지 않는 요소만 사용할 수 있다[32]. 중간 자기통합 수준에서는 Seed가 기본적으로 유지되지만 특정 관계 상황에서는 쉽게 경직화되기 때문에, 반복되는 패턴이 임상적으로 뚜렷하게 나타난다[28]. 이 단계에서는 자연스럽게 Horney–Kohut–ACT의 치료 순서가 작동한다[27]. 먼저 Horney적 탐색을 통해 환자가 자신의 관계 패턴을 안전하게 바라볼 수 있게 하고[28], 그 과정에서 드러나는 정동적 취약성을 Kohut적 공감·조율로 담아주어야 한다[30]. Seed 변형이 일정 수준에서 일어나면 방향성이 자연스럽게 떠오르며, ACT는 이 방향성을 실제 행동으로 안정적으로 이어주는 역할을 한다[33]. 이 단계는 세 이론이 가장 유기적으로 결합되는 수준이다. 높은 자기통합 수준에서는 Seed가 비교적 안정되어 있어 ACT 개입을 초기부터 적용해도 문제가 되지 않는다[32]. 다만 기능 수준이 높더라도 Seed-level의 미세한 왜곡은 여전히 남아 있을 수 있으므로, ACT만으로는 충분히 포착되지 않는 관계적 불균형을 Horney·Kohut적 개입으로 조정해야 한다[28,31]. 이러한 병렬적 조정은 행동 변화의 깊이를 확보하는 데 도움이 된다. 결국 Seed 기반 프로토콜의 핵심은 환자의 자기통합 수준을 정확히 평가한 뒤, Seed 변형이 일어날 수 있는 적절한 순서와 경험적 환경을 조성하는 데 있다. Chapter 7. Seed 변형의 임상 메커니즘 Seed 변형은 단순한 통찰이나 기술적 해석을 통해 얻을 수 있는 변화가 아니라, 환자가 세계를 경험하는 방식의 구조적 재편성을 의미한다[53]. 이는 정동이 흐르는 방식, 타자를 느끼는 감각, 자기감의 연속성이 새롭게 짜이는 과정으로, 특정 개입보다 분석가–환자 간에 형성되는 상호주관적 장(field)이 변화의 주된 매개가 된다[55]. 임상 초기 enactment는 흔히 저항이나 비건설적 행동으로 해석되지만, Seed 관점에서는 오히려 환자 경험 구조가 가장 직접적으로 드러나는 장면이다[56]. 분석가의 침묵을 ‘버림’으로 경험하거나 질문을 ‘비난’으로 느끼는 것은 잘못된 해석이 아니라 Seed가 세계를 조직하는 자동적 패턴이 작동한 결과이다[55]. 따라서 enactment는 과거를 재현하는 것이 아니라 지금 이 자리에서 다시 살아나는 관계적 경험이며, Seed 변형의 출발점이 된다. Seed 변형은 분석가가 성급한 해석이나 조절을 유보하고 모호함을 함께 견딜 때 가능하며[57], 이러한 태도는 Bion이 말한 negative capability를 임상적으로 구현하는 것이다[9]. 이 태도가 마련하는 개방된 심리적 공간에서 분석가와 환자는 기존 패턴을 반복하지 않는 새로운 관계적 리듬을 만들어내고, 이때 Ogden이 말한 analytic third가 형성된다[10]. 이러한 제3의 장은 정동·감각·관계적 움직임이 다시 조직될 수 있는 경험적 틀을 제공하며, 그 안에서 Stern이 말한 무형식 경험의 ‘함께 살아내기(living-with)’가 가능해진다[11]. 이는 Seed의 경직화된 조직 방식이 미세하게 변화하기 시작하는 핵심 순간이다. 이런 새로운 정동적 경험이 반복적으로 내면화되면서 Seed의 구조가 조금씩 다시 조직되고, 정동 폭이 넓어지고, 자기감의 연속성이 강화되며, 관계 가능성이 회복된다[58]. 결국 Seed 변형은 해석의 정확도보다 함께 견디는 경험이 더 큰 치료적 힘을 갖는 순간이며, 이 구조적 변화가 이후 Value의 자연 발생과 행동 변화의 기반을 마련한다는 점에서 임상적 의미가 크다. Chapter 8. Value의 자연 발생과 ACT의 역할 Value의 발생(Seed 변형의 현상학) Seed 변형 이후 나타나는 방향성은 인지적 선택의 결과가 아니라 경험적 생성물이다[60,61]. 이는 무엇을 해야 하는지가 아니라 ‘어디로 향하고 싶은가’라는 살아 있는 움직임이자, Seed가 재조직되면서 회복되는 정동·관계적 유연성의 자연스러운 발아이다. 이러한 방향성은 아직 행동으로 실현되기 전의 초기적 형태이며, 존재감이 확장되면서 생기는 미세한 생동성의 흐름에 가깝다. 따라서 Value는 ACT에서 다루는 치료적 구성요소에 앞서, Seed 변형이 이루어졌을 때 자연스럽게 드러나는 경험적·현상학적 생성물로 이해할 필요가 있다. Value가 행동으로 이어지지 않는 이유와 ACT의 임상적 역할 Value가 명확하게 떠오르지 않는 임상적 이유는 환자가 동기나 의지가 부족해서가 아니라 Seed 구조가 여전히 생존 중심의 경직화에 머물러 있기 때문이며[29], 바로 이 지점에서 ACT의 역할이 분명해진다. ACT의 핵심 기능은 Value를 외부에서 ‘찾아내는 것’이 아니라, Seed 변형 이후 발아한 방향성이 회피·불안·관계적 제한에 의해 좌절되지 않도록 행동적 구조를 조정하는 데 있다[32,62]. ACT의 심리적 유연성, 수용, 현재 인식, 가치 기반 행동은 변형된 경험이 일상적 삶 속에서 실현될 수 있도록 지지하는 행동 플랫폼으로 기능하며, 결국 ACT는 Seed 변형 이후 등장한 ‘존재론적 방향성’을 실제 행동으로 번역해내는 필수적 파트너가 된다. Chapter 9. General Discussion Seed 모델은 경험 발생의 최기층을 하나의 임상 단위로 재정식화하려는 시도이며, 이를 통해 기존 정신분석 이론이 다루지 못했던 ‘경험의 최초 생성 조건’을 가시화한다는 점에서 의의가 있다. Seed는 Bion의 O[9], Ogden의 analytic third[10], Stern의 unformulated experience[11]처럼 전통적으로 분절적으로 다루어졌던 표상 이전적 층위를 발달적·관계적 언어로 통합해, 경험이 어떻게 발아하고 어떻게 굳어지며 어떤 조건에서 다시 변형될 수 있는지를 하나의 구조 안에서 설명한다[53]. 이러한 접근은 임상 현장에서 자주 목격되는 “이해는 하지만 행동이 바뀌지 않는 현상”의 원인을 Seed 경직화라는 깊은 층위에서 설명할 수 있게 해 준다[29]. 즉, 변화의 부재가 의지나 동기 부족 때문이 아니라 경험 생성 구조가 여전히 생존 기반 방식에 고착되어 있기 때문이라는 관점은, 기존 해석 중심 모델과 다른 새로운 임상적 시각을 제공한다. Seed 모델은 또한 Horney–Kohut–ACT를 단순한 이론 병렬이 아니라 경험 발생의 순서에 따라 배열함으로써, 이 세 이론 사이에 존재하던 ‘단절의 문제’를 해소하려 한다. Seed가 굳어진 패턴은 Horney의 기술이 설명하는 생존 전략의 표현이고[28], Seed 변형은 Kohut의 공감적 자기대상 조율을 통해 가능하며[30], 변형 이후 생겨나는 방향성은 ACT의 실천적 기술을 통해 삶 속에서 안정적으로 유지된다[32]. 이러한 연결성은 통합 모델이 기법 중심이 아니라 경험 구조 중심의 흐름 위에서 구성되어야 한다는 점을 강조한다[27]. 다시 말해, Horney는 Seed가 ‘어떻게 작동하는가’를 알려주고, Kohut은 Seed가 ‘어떻게 변형되는가’를 설명하며, ACT는 변형된 Seed가 ‘어떻게 살아지는가’를 보여준다. 이 세 층위는 서로 대체 관계가 아니라, 발달적·경험적 연속선을 이루는 필수 구성요소다. 또한 Seed 모델은 ACT에서 빈번히 발생하는 “가치(Value)가 명확하게 떠오르지 않는다”는 임상적 난점을 설명한다. 값이 떠오르지 않는 이유가 환자의 ‘동기의 부족’ 때문이 아니라 Seed 구조가 경직된 상태이기 때문에 방향성이 자연 발아하지 못한다는 점은[60], ACT 모델을 임상적으로 더욱 미세하게 구분해 적용하게 만든다[32]. 이는 행동치료와 정신분석 사이에서 흔히 발생하는 대립적 논의—정동과 관계를 중시할 것인가, 행동 변화를 우선할 것인가—를 ‘경험 발생 과정의 서로 다른 단계’라는 관점으로 재구성한다는 점에서도 중요한 시사점을 제공한다. 그러나 Seed 모델은 초기 개념 단계에 있으며, 변화 과정이 주로 정성적·경험적 서술에 의존하고 있다는 제한점이 있다. Seed 변형의 경험적 순간은 명확하게 관찰되지만 측정과 검증이 어렵고[65], 이를 표준화된 프로토콜로 정교화하기 위해서는 체계적 사례 연구와 비교 연구가 필요하다. 그럼에도 불구하고 Seed 모델은 임상가들이 이미 체감해 온 “변화가 일어나는 자리”를 보다 분명한 언어로 설명하며, 정동·관계·행동의 통합적 구조를 제시한다는 점에서 유의미한 기여를 한다. Chapter 10. Conclusion 본 연구는 다양한 임상 현상 아래에 존재하는 보다 근원적인 경험 구조를 ‘Original Seed’라는 개념으로 재정식화하였다. Seed는 초기 조율 경험 속에서 형성되는 정동·감각·관계적 리듬의 발생 구조이며[24,25], 이 구조가 생존 중심 방식으로 경직화될 때 정동 협소화, 반복적 관계 패턴, 자기감의 취약성이 나타난다[15,28]. 이러한 관점은 많은 환자들이 인지적 이해나 통찰이 있음에도 불구하고 행동 변화로 나아가지 못하는 이유가 의지의 문제가 아니라 경험 구조의 경직화 때문임을 설명해준다[29]. Seed 모델의 핵심 기여는 변화가 일어나는 ‘자리’를 기존의 해석·인지 중심 모델에서 벗어나, 상호주관적·정동적·발생적 층위로 이동시켰다는 점에 있다. Seed 변형은 성급한 의미화가 아니라, 치료 장면에서 재현되는 조율 실패의 순간을 분석가가 환자와 함께 견디는 과정 속에서 일어나며[57], 이때 Bion의 negative capability[9], Ogden의 analytic third[10], Stern이 말한 unformulated experience의 공동-살아내기(living-with)[11]가 임상적으로 결합된다. 이러한 새로운 경험의 축적이 존재적 방향성(Value)의 자연 발아를 가능하게 한다[60,61]. 또한 본 연구는 Horney–Kohut–ACT를 경험 발생의 층위에 따라 재배열함으로써, 이 세 이론이 경쟁 관계가 아니라 하나의 연속선 위에서 서로 다른 역할을 수행함을 밝힌다. Horney는 Seed 경직화가 임상에서 어떻게 표현되는지를 지도화하고[28], Kohut은 Seed 변형이 일어나기 위한 정동적 조건—공감적 자기대상 경험과 안정화—을 제공하며[30,31], ACT는 변형된 방향성이 회피와 불안으로 좌절되지 않고 행동으로 구현될 수 있도록 행동적 구조를 조정한다[32,62]. 이러한 통합은 정동·관계·행동을 하나의 임상적 흐름으로 연결하는 새로운 치료적 패러다임을 제시한다. 향후 연구는 Seed 경직화와 변형 과정을 객관적 지표로 탐색하고[65], 다양한 임상군에서 Seed 기반 프로토콜의 효과를 비교하는 방향으로 확장될 수 있을 것이다. 그럼에도 본 연구는 임상가들이 반복해서 마주하는 질문—“왜 사람은 알고도 변하지 않는가?”—에 대해, 경험 발생 구조의 가장 깊은 층위에서 답을 제시한다는 점에서 의미가 있다.

    이창훈 2023-07-23 초 록 환자의 마음에 보다 더 닿을 수 있는 말을 하기 위한 고민은 모든 정신분석가들의 숙명이다. 분석가의 말이 환자의 심리적 현실과 동질적일수록 환자의 마음에 잘 스며들겠지만, 그런 말은 환자의 심리적 현실을 변화시키기 어렵다. 환자의 심리적 현실에는 이질적이지만 만약 그 해석이 환자의 마음에 닿아 마음을 움직일 수 있다면 환자의 심리적 현실에 변화를 일으킬 수 있을 것이다. 환자의 심리적 현실에 변화가 일어나도록 돕는 것이 분석가가 하는 일이 아닌가? 어떻게 환자의 심리적 현실과는 이질적인 분석가의 해석이 환자의 마음에 잘 닿게 할 수 있을까? 나는 환자의 마음에 닿을 수 있는 분석가의 말에 대한 두 관점을 구분하고 비교할 것이다. 이 두 관점은 대조적이다. 첫번째는 환자의 마음에 잘 닿기 위해서 치료자의 해석은 환자의 마음에 동질적이면서 서사적이어야 한다는 관점이다. 두번째는 환자의 마음에 이질적이지만 환자의 마음을 변화시킬 수 있는 해석이어야만 정신분석의 궁극적인 목표를 이룰 수 있다는 관점이다. 나의 증례를 통해 이 두 관점을 비교하여 구체적으로 설명하고, 환자의 마음을 변화시킬 수 있는 해석을 하기 위해서 프로이드의 구조이론과 저항에 대한 개념이 왜 중요한지를 되짚어볼 것이다. 키워드 : 해석, 저항, 서사적 진실, 심리적 현실, 이질적 환자에게 뭔가 이야기하려고 할 때, 내 마음 속에 떠오르는 그림이 있다. 내 말이 환자의 마음에 들어가려고 하는 장면이다. 그런데 마치 세포막이 세포를 감싸고 있듯이 환자의 마음도 어떤 막으로 싸여 있어서 내 말이 그 막을 통과하기 위해서는 그 막의 수용체에 딱 들어맞는 요소를 가지고 있어야 한다. 이 생각은 오래전부터 나에게 있었다. 환자 마음의 막을 부드럽게 통과하기 위해서는 분석가의 말에 어떤 요소가 필요할까? 그것을 갖추지 못한다면 분석가의 말이 환자 마음에 닿을 수가 없을 것이기에, 환자와 분석가의 대화는 무의미한 것이 될 수 밖에 없을 것이다. 이에 대한 연구는 효과적인 해석에 대한 연구에서부터 분석의 진정한 목적이 무엇인가하는 것, 궁극적으로 정신분석적 진실이 무엇인가 까지를 아우르는 매우 방대한 연구가 될 것이다. 하지만 이 글에서는 환자의 마음에 잘 닿을 수 있는 분석가의 말에 대한 두가지 관점을 구분하는 것에 초점을 맞출 것이다. 서로 다른 이론적 배경을 가진 정신분석가들이 공통적으로 중요하게 여기는 것 중에 하나는 ‘어떻게 해야 나의 말이 환자 마음의 중심부에 가서 닿을 수 있을까?’ 하는 것이다. 분석가의 말이 환자 마음의 중심부에 닿는다는 것은 비유적인 표현이다. 환자 마음의 중심부란 겹겹이 싸고 있는 모든 방어층을 통과하여 이르는 환자의 정신 구조의 근간을 의미하는 것이고, 그곳에 닿는다는 것은 그 영역에 영향을 미쳐 좋은 변화를 일으킨다는 의미일 것이다. 프로이드의 말을 빌리자면, “자아에 이로운 변화를 일으킨다(it brings about an advantageous alteration of the ego…; Freud 1940, p179)”고 말할 수 있겠다. 프로이드는 환자의 방어를 저항이라는 개념으로 이해하고, 환자가 방어벽을 포기하고 열도록 이 저항을 끈질기게 분석하는 것을 정신분석의 목표로 두었다(Freud 1936, Busch 2014). 그러나 겹겹이 싸고 있는 모든 ‘방어층’을 통과하는 문제, 즉 저항을 끈질기게 분석하는 문제에서 분석가들은 좌절을 경험하게 된다. 환자는 자신만의 심리적 현실을 지키기 위해 겹겹이 방어벽을 쌓고 있기 때문에, 분석가의 말이 자신의 심리적 현실을 흔들 위험이 있을 때는 철저하게 막게 마련이다. 20세기 중반에 이르러 정신분석가들은 전혀 다른 방향으로 이 문제를 해결하려고 했다. 방어, 저항 이런 것을 다 잊고, 정신분석의 모든 것은 단지 서사적 진실에 바탕을 둘 뿐이라는 혁신적 발상을 하게 된 것이다(Wallerstein 1982). 이는 과학주의에 대한 반성과 회의로 인해 발달해서 20세기 초 서양문화를 휩쓸었던 현대해석학의 영향이 크다(Gedo 1997). 스펜스(1982)는 구조이론이나 무의식에 관한 이론과 같은 정신분석의 이론들부터 정신분석에서 다루는 모든 것들(저항, 무의식, 꿈, 전이 등)은 단지 “서사적 진실”의 일부이며 객관적인 타당성이 없다고 주장했다. 그에 의하면 인간의 정신을 탐구하는 정신분석은 결코 역사적(과학적) 진실을 추구할 수 없다. 정신분석적 해석은 과학적 사유로서 타당성을 추구하는 것이 아니라, 환자에게 새로운 관점을 열어주는 "서사"를 만드는 것이며, 정신분석의 목적은 환자가 자신의 삶을 새로운 관점으로 이해할 수 있는 서사를 만들어가는 것이라고 했다. 이 주장은 시대와 문화의 요구에 영향을 받고 있던, 또 환자들의 저항 앞에서 좌절하던 정신분석가들에게 상당히 설득력이 있었던 것 같다. 현재 그린버그(2015)와 같은 분석가들은 정신분석 자체가 허구로 이루어졌고, 정신분석의 치료적 요소는 그 허구를 통제하는데 있다는 다소 과격한 주장을 하기에 이르렀다. 환자의 서사를 확장시킬 목적으로 하는 해석은 환자들이 잘 받아들일 수 있었고, 정신분석가들은 환자들과 쉽게 새로운 서사를 창조할 수 있었다. 예를 들면, 환자가 “나의 부모님은 내가 최우선이 아니었어요. 항상 자기들이 우선이었고 나는 그들의 트로피 같은 존재였어요”라고 자신의 서사를 이야기할 때, 분석가는 환자와 좀더 대화를 이어간 다음, 이렇게 더 확장된 서사를 이야기하는 것이다. “당신은 지금도 헌신적으로 당신을 위하는 사람이 없다는 것이 견딜 수 없이 고통스러운 것 같네요. 그들의 트로피가 되는 것보다는 차라리 자신을 망가뜨려서 그들의 트로피를 부숴뜨리는 게 덜 고통스러운 것 같아요.” 그러면 환자는 “맞아요! 그게 바로 내가 나를 망가뜨리는 이유인 것 같아요”라고 이 확장된 서사를 환영하며 받아들인다. 이때 두 사람이 나누는 대화를 보면, 정신분석가는 환자의 감정을 정확하게 이해했고 공감적이면서도 옳은 해석을 한 것으로 보인다. 더구나 환자의 자기파괴적 행동에 대한 새로운 관점을 제시했고 환자는 이에 대한 이해를 한 것 같이 보인다. 한때 나는 이러한 서사적 진실을 바탕으로 한 서사적 해석이 환자 마음의 막을 부드럽게 통과하여 마음의 중심에 닿을 수 있게 하는 요소라고 믿었다. 이것이 환자 마음의 방어벽을 쉽게 뚫고 환자에게 받아들여지게 하는, 즉 환자의 저항을 극복하게 하는 매우 유용한 열쇠라고 생각했다. 그러나 이후 나는 큰 그림을 못보고 있다는 것을 깨달았다. 서사적 해석이 환자의 마음에 쉽게 전달될 수 있는 것은 맞으나, 저항이 극복된다는 것과는 별개의 것이다. 즉, 위의 예에서 보면, 환자가 자기파괴적인 행동에 대해 이해한 것처럼 보이지만, 환자의 심리적 현실이 전혀 변화하지 않고도 환자가 이해는 할 수 있다. 이 환자는 자신의 행동이 자기파괴적이라는 깨달음보다는, 자신의 자기파괴적 행동의 원인이 외부에 있다고 믿는 자신의 심리적 현실을 다시 한번 확인하는 것 같다. 스펜스는 “새로운 관점을 열어주는 서사”에 대해 구체적인 논의를 하지 않았다. 단지 그는 “그럴 듯하고, 친숙하며, 자주 일어나는” 이야기라면 나름의 진실을 품고 있다고 주장했다 스펜스(1982). 그러나 나와 주변 동료들의 경험에 비추어 볼 때, 환자의 심리적 현실을 변화시킬 수 있으려면, “그럴 듯하고, 친숙하며, 자주 일어나는” 정도의 이야기로는 절대 불가능하다. 서사적 해석은 환자의 마음과 동질적인 요소를 포함하기 때문에 쉽게 환자 마음에 닿을 수 있는 것이 당연하다. 저항을 극복한다는 것은, 막 비유를 사용하자면, 그 막의 수용체 자체가 변화하는 것이라고 하겠다. 환자의 마음에 이질적인 해석이 마음속으로 들어가려면, 먼저 동질적인 것만 받아들이던 수용체가 이질적인 것도 받아들일 수 있게 변화되어야 한다. 어떻게 해야 그렇게 될 수 있을까? 그런데 이 중요한 저항에 대한 작업, 저항 분석에 대한 연구가 그리 활발하지 않았다 (Busch 2014). 그레이(1982)는 프로이드가 저항의 완고함을 표현한 것을 빌려, 분석가들 역시 저항 분석 -저항을 인식하고 개념화하는- 에 대한 저항이 있다고 완곡하게 지적했다. 아마도 그는 저항 분석과 철저 작업을 잊고 정신분석의 모든 것이 서사의 일부라는 해석학적 관점으로 쏠리는 분석가들에게 일침을 주고자 했을 것이다. 나는 환자 마음에 깊이 닿을 수 있는 치료자의 해석에 대한 정신분석가들의 관점이 크게 두 방향으로 나누어져 있다는 것을 깨달았다. 첫번째는 서사적 해석과 같이 환자 마음에 동질적일수록 환자의 마음에 깊이 닿을 수 있다는 관점이다. 그러나 이 관점은 이것은 환자가 바로 받아들이지만 그의 심리적 현실에 변화를 만들어내기가 힘들다는 것을 간과하고 있다. 두번째 관점은 환자가 변화하도록 돕기 위해서는 분석가의 해석이 필연적으로 환자의 심리적 현실에는 이질적일 수 밖에 없다는 것이다. 이질적이면서도 환자의 저항에 변화를 일으키고 환자의 마음을 움직이는 해석이어야만 진정으로 환자의 마음에 깊이 닿을 수 있는 해석이라고 할 수 있다. 나는 이 두가지를 구분하는 것이 중요하다고 생각한다. 왜냐하면 현대정신분석에서 서사적 진실과 서사적 해석을 무분별하게 사용하고 있고, 이로 인해 정신분석의 핵심이 많은 정신분석가들의 관심에서 잊혀지고 있기 때문이다 (Blass 2015). 해석학적 정신분석가의 대표라 할 수 있는 쉐이퍼(2005, p62)는 자신의 임상 여정이 거의 끝나갈 무렵에 이렇게 말했다. “정신분석적 개입은 환자의 서사를 개작(retelling)하는 것이다. 이를 통해 환자에게 이로운 분석적 변화를 촉진할 것이라는 ‘어떤’ 약속을 할 수 있는데, 이 변화는 자아가 갈등을 다룰 수 있게끔 준비시키는 깨달음(insight)과 철저작업(working through)에 의해 이루어진다. 내적 갈등의 서사를 무분별하게 사용할 때 일어날 수 있는 이론적, 기법적 부작용에 대해서는 경계하지만, 그럼에도 나는 환자의 서사를 개작(retelling)하여 내적 갈등을 이야기하는 것을 여전히 선호한다 (Schafer 2005, p62, ‘ ’저자 삽입)”. 나의 편견일지 모르나, 그의 문장에서 서사적 해석이 환자의 변화를 일으킨다는 약속을 미묘하게 보류하면서, 깨달음과 철저작업에 그 책임을 미루는 것이 느껴진다. 무엇에 대한 깨달음인지, 무엇을 철저작업해야 하는지는 언급하지 않아서 사실상 어떻게 변화가 가능한지 설명하지 않았다. 더구나 그는 간접적이기는 하나 서사를 무분별하게 사용할 때 일어날 수 있는 이론적, 기법적 부작용이 잠재해 있다고 언급하고 있다. 그러나 어떤 부작용이 있는지 구체적으로 설명하지 않았다. 결국 그의 글은 역설적이게도 서사적 해석을 통해서는 깨달음과 철저작업이 어렵다는 것을 시인하는 것처럼 들린다. 서사를 무분별하게 사용할 때 우려되는 부작용이 무엇인지에 대해서 나의 견해는 다음과 같다. 프로이드의 구조이론이 의미를 잃게 되고, 갈등, 저항의 개념이 사라지게 된다는 것이다.이렇게 되면 정신분석은 치료가 아니라 두 사람간의 특정한 양식의 대화가 될 뿐이다. 이제 증례를 통하여 두 관점의 해석이 어떻게 다른지에 대해서 자세히 다룰 것이다. 이 글은 두가지 다른 관점의 해석을 구분하는 것에 초점을 두고 있으므로 그 이상의 많은 논의가 있을 수 있겠지만 이번 글에서는 다루지 않을 것이다. 다음 증례는 저자가 분석치료한 환자의 증례이며, 발표하도록 허락해준 환자에게 감사를 드린다. A의 분석 증례 Confidential Case material to non-Member 위의 증례의 한 장면을 통해서, 어떻게 환자의 마음에 닿을 수 있는 해석을 하는가에 대한 두가지 다른 관점을 볼 수 있다. 첫번째는 동질적인 서사적 해석으로 환자가 쉽게 받아들일 수 있다. 서사는 주로 대인관계 측면으로 구성되고(Gedo 1997), 감정을 부각시키고, 환자의 마음에 동질적이다. 서사적 해석이 환자의 마음에 쉽게 받아들여지는 것은 그것이 환자를 만족시키기 때문일 것이다(Spence 1982). 서사의 확장과 새로운 서사의 창조가 정신분석의 목표라고 한다면 환자를 만족시키는 것이 정신분석의 목표라고 하는 것과 같다. 우리는 정신분석은 환자를 만족시켜서 가능한 것이 아니라는 것을 잘 알고 있지 않은가? 새로운 서사를 통해 환자의 관점이 변화할 수 있을 것이라는 희망을 가질 수도 있겠지만, 서사를 통해 환자가 자기 자신을 성찰하도록 하는 것이 생각보다 쉽지 않다. 환자를 만족시키지 않는 서사는 일단 환자에게 채택되지 않기 때문이다. 그리고 내적 갈등은 서사를 통해 잘 표현할 수가 없다. 서사는 대인관계 측면으로 구성되는 특징이 있다고 했는데, 대개는 환자의 내적 갈등이 외부로 투사되어 표현되기 마련이다. A의 서사에서 A가 더러운 타르가 세상에서 묻혀온 것이라고 하듯이 말이다. 두번째는 이질적이지만 변화를 일으키는 해석이다. 이것을 저항에 초점을 맞춘 서사적 해석이라고 바꿔 말할 수 있다. 프로이드가 중시했던 저항에 대한 끈질긴 분석이 어떻게 환자에게 효과적으로 받아들여질 수 있는가 하는 질문에 대한 나의 답이다. 나는 정신분석가들이 환자의 서사를 경청하면서 반드시 저항의 측면을 염두에 둘 것을 강조하고 싶다. 그리고 환자의 저항이 치료과정을 방해하는 것이 너무 공고해지기 전에 저항에 초점을 맞추어 서사적 해석의 방향을 바꾸어야 한다. 이에 대해 부쉬(2014)는 정신분석가들이 프로이드의 저항과 철저작업에 대한 관점을 잃지 않기를 염원하면서, 저항분석이 철적작업의 핵심이며 이를 통해 지속되는 견고한 변화를 이루어낼 수 있다고 역설했다. 그는 어떻게 환자가 저항 분석을 통해 자신의 무의식에 닿을 수 있는가가 정신분석 기법에서 가장 중요한 문제라고 했다. 환자의 서사는 환자의 심리적 현실을 반영할 수 밖에 없다. 정신분석가의 해석이 환자의 심리적 현실을 확장하는 것을 부추키는 것이 되어서는 안된다. 환자의 서사를 경청하여 그의 심리적 현실을 충분히 이해한 이후에는, 자신의 심리적 현실을 유지하려는 환자의 저항과 맞서야 한다. 환자의 서사에서 이용되는 저항을 깨닫고 구체적으로 찾아내기 위해서는 환자의 서사를 구조이론에 대입하여, 이른바 새로운 서사를 만들어낼 수 있어야 한다. 환자의 욕구와 두려움, 내적 갈등의 구조, 방어체계등의 포괄적 이해가 있어야 환자의 서사를 관통하여 바라볼 수 있다. 그리하여 환자가 사용한 서사적 틀을 이용하여 환자에게 받아들여질 수 있으면서도 변화를 일으킬 수 있는 해석을 할 수 있다. 해석학적 정신분석가들은 새로운 서사의 창조에 대해 구체적인 방향을 제시하지 못했다. 말그대로 새롭기만 하면 모두가 다 진실을 담고 있다는 믿음은 쉐이퍼(2015)의 말을 빌리자면 “무분별하게 서사를 사용”하게 하고 “이론적, 기법적 부작용”을 낳게 된다. 쉐이퍼가 이 말을 서사적 해석을 분별력있게 사용하자는 의미에서 한 것은 아니고, 그런 우려도 있지만 자신은 서사를 매우 중요하게 여긴다는 뜻에서 한 것이지만 말이다. 나는 새로운 서사의 창조가 정신분석의 근본 위에서 이루어져야 한다고 믿는다. 정신분석의 근본이 무엇인지를 따진다면 또 다시 거대한 논쟁이 시작되겠지만, 내가 말하는 정신분석의 근본은 프로이드의 구조이론을 토대로 한 방어와 저항 분석에 초점을 맞추는 것을 의미한다. 맺음말 나는 해석학적 정신분석가들이 새로운 서사의 창조, 환자와 함께 창조하는 것을 강조하는 것에 궁극적으로 동의한다. 하지만 환자와 함께 창조하는 새로운 서사는 환자가 이전에는 전혀 알지 못했던, 알고 싶지 않았던 서사를 창조할 수 있도록 정신분석가가 도와야 한다고 생각한다. 환자는 알지 못하거나 알고 싶지 않은 것을 아는 것을 저항하며, 자신이 알고 있는 세계를 지키려고 방어한다. 비욘(1962)은 분석가의 해석은 환자가 생각하지 못하는 것을 생각할 수 있도록 돕는것이라고 했고, 옥덴(2004)은 꿈꾸지 못하는 것을 꿈꿀 수 있도록 하는 것을 분석이라고 했다. 이를 위해서는 필연적으로 “저항”을 분석해야한다는 것이 나의 주장이다. 환자의 저항은 구조이론적 관점에서 바라볼 때 명확하게 보여진다. 환자와 함께 창조하는 새로운 서사는 구조이론에 바탕을 두고 환자의 서사에 자리한 저항을 발견하여 이를 환자가 이해할 수 있도록 돕는 서사를 함께 만들어가는 것이라고 생각한다. 참고문헌 Bion W (1962) Learning from experience, chapter twenty-six, London: Karnac.   • Phenomenolgy, Hermeneutics, and Wilfred Bion’s Theory of the Primitive Mental States

    손홍석 2025-01-01 1.Phenomenology, Hermeneutics, and Psychoanalysis While the discipline of hermeneutics and psychoanalysis align with or coincide, the relationship between phenomenology and psychoanalysis is conflictual or, at least, complex. As Ricoeur demonstrated, psychoanalysis can be seen as a mode of hermeneutics: the hermeneutics of desire in suspicion of the patient’s discourse, compared to that of the text in hermeneutics.[1] According to him, the purpose of psychoanalytic theory, as the work of interpretation within the region of desire, lies in setting out the conditions of possibility of a semantics of desire. Ricoeur finds similarity between phenomenology and psychoanalysis in the respect that they share the premise that the meaning and the origin of language is incarnated in the body, and in their coincidence of theory of intersubjectivity in the constitution of the subject.[2] When psychoanalysis starts to demand the use of the unconscious as a substantive meaning, however, it falls to the category of “an epoché in reverse,”[3] an anti-phenomenology. Phenomenology’s fundamental hold on the most known consciousness contrasts with psychoanalysis’ seeking of the least known unconscious. The phenomenological investigation would clear out what does not manifest itself, the un-conscious. Nevertheless, paradoxical relationships arise when phenomenology pushes itself to the limit towards the revelation of what is beyond the objectified and represented intentionally, i.e., as in phenomenology after Heidegger, Merleau-Ponty, and Levinas. Then, “a phenomenology of unconsciousness pushes both the conception of the phenomenon and the conception of logos to their limits, or beyond.”[4] Therefore, phenomenology and psychoanalysis merge again in the domain of the inaccessible. The inaccessible, firstly, in the sense of “transcendent,” which can never become accessible to human beings. Secondly, in the sense of “limitation,” which disallows the human to apprehend it as it is in-itself.[5]Unfortunately, the fruitful philosophical studies on the crossover of phenomenology and psychoanalysis have, thus far, been concentrated only to the narrow extent of psychoanalysis, mostly and limited to that of Freud and Lacan.[6] 2. Wilfred Bion’s Theory of the Primitive Mental States: The Phenomenology of the Absence In contemporary psychoanalysis, one of the significant trends has been the shift toward the practice and theoretical explorations of primitive mental states. While Freud conducted classical psychoanalysis on patients with neurotic problems, the object-relations theory and infant studies took a step inside the deeper level of pathologies, thereby gaining knowledge of the infantile lives. Nowadays, psychoanalytical encounters involve patients struggling with even more severely compromised psychic structuralization deriving from deeper levels and earlier stages of life: psychotics, severe trauma, and autistics. Such trend unfolding the studies of primitive minds rely heavily on Wilfred Bion. Bion’s work made it possible to explore the primordial mental state, unmapped by the previous theories. Psychoanalysis of the neurotic conflict and object-relations elaborates the mind as premising the capacity for representations - dream thoughts, symbols, and fantasies. Bion’s psychoanalytic theory, on the other hand, questions the fundamental possibilities and conditions of any representation in the mind. Primitive mental states are devoid of representational and symbolic function or any differentiation between psyche/soma, subject/object. Bion could not but postulate that the mental space or the human psyche is “a thing-in-itself, that is infinite and unknowable.”[7] His scrutinization of the unrepresented states,[8] or the “ineffable experience unmediated by thought,”[9] is phenomenological in that he tried to investigate the foundational possibility of the thinking process itself and his careful elucidation of experience without any suppositions. Bion starts with the phenomenology of the clinical situation, focusing on the inner processes of the mind, where he examines its constituent elements.[10] This phenomenological stance drives Bion’s later works to delve into the realm of the ultimate reality, moving away “from mechanism and causality and toward a phenomenology of what is” of which dimension is “beyond psychoanalysis to draw on philosophical and mystical ideas to impart a mode of apprehension that is nonlinguistic.”[11] In short, one can denote Bion’s psychoanalytical investigation of the primitive mind as the “phenomenology of the psychosis” in clinical terms,[12] where he sought to explore the preconditions of achievement of a capacity for verbal thought. Furthermore, Bion’s work can be designated as the “phenomenology of the absence” in theoretical terms,[13] concerning the fact that he grounded the possibility of thinking under the condition of the loss or absence of the object. Bion assumed that the capacity to think can be achieved “when the absent object is recognized as absent but continuing to exist in some other [mental] space.”[14] 3. The Unstructured Unconscious, the Unrepresented States, and the Representational Imperative “Unstructured unconscious” and “unrepresented states” are the recently conceptualized terms in the most contemporary - the 21st century - psychoanalytic discourse to denote the above-mentioned primordial or traumatized mental states.[15] he unstructured unconscious denotes the unstructured and unintegrated flows of forces in a formless state derived from and inscribed in the soma, never capable of being retrieved as a memory nor of being expressed in language.[16] Experiences in this realm are unattainable in mental representation; thus, unlived experiences that cannot be said to have been experienced. It must be transformed within the intersubjective context with the concrete Other to be relived and reclaimed. The intersubjective processes through which representations become formed, strengthened, connected into narrative fragments and associative chains, connected to stable affects, and linked together in the pairs symbol and symbolized, signifier and signified; and the structuring role of language in the process of effective, emotionally invested thinking and the creation of mind.[17] “The representational imperative” is a psychic activity “governed by an inherent pressure to form representations and link them into meaningful, affect-laden, coherent narratives.” This demand upon the mind is double-sided. When the pressure is contained within the optimal bounds, it has a vital protective role in the organization of the mind. On the other hand, the exceeding becomes “traumatic.”[18] ※ Argument In psychoanalytic theories, the carnal desire to know is signified as “epistemological instinct(drive)” and “representational imperative.” Moreover, the concrete Other is designated as a “container” that co-constructs the understanding of the excess and the incomprehensible in an intersubjective context. Thus far, as explained in detail, the psychoanalytic theories of the primitive mind explore the constitutional elements that shape the human experience and affect human response to the phenomena as they are given.   • Wilfred Bion and The Language of Achievement

Limitation of language in psychoanalysis The very limitation of language becomes apparent in every failure to convey its essence in the universal communication of emotional experiences in any psychoanalytic dialogue. The deeper the psychoanalytic explorations move forward, the dilemma becomes more pronounced: the dilemma between the verbal and preverbal, or the phenomenal world and the numinous world. It arises from the simple fact that “not only is the human mind a vast internal universe that far outstrips our capacity to understand it, but analysts rely heavily on verbal language that often must be used to express pre-verbal mental states.”[1] Bion asserted that the insufficiency of language was fundamental aporia in psychoanalysis by noting that the “problem presented by psycho-analytic experience is the lack of any adequate terminology to describe it.”[2] The problem becomes more relevant when approaching a mental life “unmapped” by the previous theories elaborated for the neurosis,[3] especially in the respect that the unstructured and unsymbolized aspects of experience are literally unthinkable.[4] Working with the patients who are severely compromised in psychic structuralization, Bion could not but postulate that the mental space or the human psyche is “a thing-in-itself, that is infinite and unknowable, but [nevertheless] that can be represented by thoughts.”[5] Thus, the discrepancy is always laid between the thing-in-itself or the existential realm of the raw emotional life and what can be represented in mind or known to be experienced, and finally what can be put into verbal thought or conveyed in words. 2. The Language of Achievement Amid the seeming impasse, Bion conceives of the Language of Achievement as not the most sufficient but at least the proximate language used for the ineffable emotional interaction and communication in the psychoanalytic experience – the communication between the analysand and the analyst, or that between the analyst and other analysts. He coins the term from Keats “Man of Achievement” and “Negative Capability.”[6] I had not a dispute but a disquisition with Dilke on various subjects; several things dove-tailed in my mind, and at once it struck me what quality went to form a Man of Achievement, especially in Literature, and which Shakespeare possessed so enormously—I mean Negative Capability, that is, when a man is capable of being in uncertainties, mysteries, doubts, without any irritable reaching after fact and reason. – John Keats.[7] Bion contrasts the Language of Achievement with the language of substitution.[8] The latter denotes language that is solely sensuously derived, based upon representations of objects and substitutive symbols. The unsuitability of these ordinary languages to convey ineffable experiences makes requesting other sorts of language necessary. Therefore, the Language of Achievement is “the language of emotions before they become represented as concepts or ideas—and it also is the language of models—that is, analogue models and experiences outside the system under investigation.”[9] The Language of Achievement, according to Bion, is employed for elucidating the truth revealing itself from the communications “deriving not only from sensuous experience but also from impulses and dispositions far from those ordinarily associated with scientific discussion.”[10] Bion places this language in the domain where there is no time or space, where one is open to the non-sensuous infinite realm of the unconscious emotional life.[11] This mean that the Language of Achievement indicates the capacity to contain the primitive emotional life. In contrast, the language of substitution indicates the defensive stance that obstructs real communication.[12] Lies – e.g., illusions, omnipotent fantasies, and delusions – substitute the truth when one is intolerable of frustrations derived from primitive emotions and unable to contain those feelings – not only of love but also hatred, envy, and greed that dominates the earliest period of life and analytic sessions.[13] The growth or the development into mature personality is acquired through not the preclusion of these cruel psychic realities but the transformations of the fragmented into the whole through intersubjective exchanges. The Language of Achievement is transformative in its nature and sought for an activity that is both the restoration of god (the Mother) and the evolution of god (the formless, infinite, ineffable, non-existence), which can be found only in the state in which there is NO memory, desire, and understanding.[14] According to Grotstein, Christian mystics’ description of apophatic mystical language of unsaying deemed “so as not to defile the essence of the deity by capturing him in language.” – as in Meister Eckhart’s notion that any spoken word must be followed by its opposite.[15] He further designate the Language of Achievement as the language of paradox in that the numinous would never fall into the category of object of which can be captured by any language. The unsaying and the dialectical procedure are required to preserve the deity’s vitality and transcendence.[16] It appears plausible that Bion borrows the term from the language suitable for literature or religion in a struggle to convey the experiences in a not yet represented psychic level. It must have had to be the language in contact with the a-sensous and undifferentiated reality – of which Bion terms the ultimate O - and evokes psychic change. Bion acknowledged the similarity of the realm that poetic or religious language touches and that of the psychoanalytic one and, moreover, intended to produce a poetic anthology for the analyst in his late and autobiographical works.[17] However, he was more aware of the danger in using poetry to convey psychoanalytic experiences, in a sense that the artist’s capacity may provide a substitute for the truth – a warning already made by Plato.[18] According to Vermote, the beauty of a poetic and religious approach may become a substitute for the truth rather than an apprehension of it. Bion never renounced apprehending the ultimate reality but altered the mode of understanding as becoming or being at-one-ment. What Bion maintains is a radical attitude, “adopting a stance which eschews understanding, and does not look for coherence, but rather attempts to be in touch with an undifferentiated zone from where thoughts arise.”[19] [1] Annie Reiner, “What language are we speaking?     이창훈 2023-07-23 1. GPT 기술을 활용한 챗봇(채팅 로봇)에 대한 논의는 2020년 이후 전세계인들을 가장 흥분하게 하는 주제 중에 하나이다. 2. GPT 기술은 인공지능 컴퓨터가 온라인 상에 존재하는 수집 가능한 모든 정보를 딥러닝하여 사용자의 요구에 최적합한 대답을 내놓을 수 있게끔발전하고 있는 중이다. 따라서 정보나 지식에 기반한 문제 해결을 위한 질문에는 현재의 GPT 챗봇도 충분히 만족할 만한 답을 내놓고 있다. GPT 개발 회사인 openai.com에서 제공하는 챗봇에게 요구를 하고 그것이 답하는 것을 읽고 있으면 마치 저 멀리 어딘 가에 존재할 것만 같은 아주 똑똑한 누군가와 대화를 하고 있는 것 같다. 정말 사람과 대화하고 있는 기분이 든다는 말이다. 실제로 최신 버전인 GPT-3R이 이전 GPT-3과의 차별점은 사람처럼 대화한다는 것이다. 그전의 ‘기계’스러움을 벗어나 실제 사람과 대화하는 것과 같은 느낌을 느낄 수 있는 반응방식을 습득했다고한다. 그렇다면 정보나 지식에 기반한 문제 해결만이 아닌, 인간의 감정과 무의식에 기반한 문제까지도 GPT 챗봇이 적절한 혹은 최적의 답을 할수 있을 만큼 기술이 발전될 수 있을까? 만약 그렇게 된다면 GPT 챗봇이 정신분석가의 역할을 어느 정도는 대체할 수 있는 때가 오지는 않을까? 하는 두려운 질문이 저자의 마음 속에 생긴다. 저자는 이에 대해 논의해보려고 한다. 3. 우선 GPT 챗봇의 원리를 알아볼 필요가 있다. 매우 간단하게 GPT 기술의 원리를 설명해보면, 이 기술은 수집한 모든 정보를 딥러딩한 후 사용자의 요구에 대해 가장 확률적으로 적합한 답을 하는 것이다. 수집한 정보를 바탕으로, 확률적으로 적합한 답을 하는 것이므로, 수집한 정보에 따라답이 달라질 수 있고, 수집한 정보 중에서 가장 적합할 확률이 높은 답을 하게 된다. A. 여기에서 GPT 기술이 미래의 정신분석가의 역할을 대신할 수 있을까 하는 질문에 대해 내가 “아니요”라고 대답할 수 밖에 없는 첫번째 이유가 설명된다. GPT 챗봇은 개개인의 특별한 경험과 상황에 맞추어 반응할 수는 없다. 만약 그것이 가능하려면 한 개인에 대한 방대한 정보를추가로 입력하고 딥러딩을 시킨, 각 개인에게 개별화된 GPT가 개발되어야 할 것이다. 그러나 그것은 거의 불가능하다고 생각된다. 각 개인의 방대한 정보는 결국 개인 각자가 입력해야 할 것이고, 그것을 딥러딩 시키기 위해서는 막대한 비용이 필요할 것이다. 그 비용은 분석가에게 지불하는 비용과는 비교도 되지 않을 것이다. 수십년 이후에 기술이 비약적으로 발전하여 딥러딩에 드는 비용을 획기적으로 낮추게 된다면 가능할 수도 있겠다. 그러나 분석치료를 필요로하는 환자가 자신의 정보를 컴퓨터에 일일이 입력할 것이라는 것은 상상하기가 어렵다. B. 두번째는 모든 사람들이 가장 많이 제기하는 한계점이다. 인간의 감정을 학습시킬 수 있는지에 대한 의문이다. 인간의 감정적 반응을 확률적으로 학습시킬 수 있다고 하더라도 (물론 상상을 초월하는 비용이 들것이다) 인간의 무의식적 심리과정을 어떻게 학습시킬 수 있는가? 이것은 교육분석가들조차도 분석훈련생들에게 교육하는데 한계가 있고 매우 힘든 과정이다. 이 교육 과정에 대한 정신분석학의 연구도 충분하지 않으며 여전히 연구되어야 하는 분야이다. C. 세번째 부정적 예상이 되는 이유는, 확률적으로 최적합한 답을 하기 때문이다. 기계의 확률이란 가장 흔하고, 가장 일반적이고, 가장 평범한답이라는 의미일 것이다. 정신분석 과정에서 분석가의 반응이 환자에게 가장 도움이 되는 순간, 다시 말해 환자의 마음을 움직이고 변화시키는 순간은 분석가의 반응이 합리적이면서도 환자가 전혀 예상하지 못한 반응을 보일 때이다. 이런 순간에 환자의 기존의 심리적 현실은 평형이 깨지고 새로운 관점이 열리게 된다. 그러나 GPT 챗봇은 확률적으로 최적합한 답을 하기에 바로 그 환자만의 마음에 최적합한 반응을 보이는 것은 매우 어려울 것이다. D. 네번째는 정신분석의 역사에서 점차 환자와 치료자의 상호관계와 치료자의 역전이를 이해하는 것이 중요해지고 있는 것과 연관된 이유이다. 현대 정신분석은 환자의 마음만을 거울처럼 관찰하는 것이 아니라, 분석가와 환자의 상호관계에서 일어나는 현상을 이해하고, 새로운 서사와 의미를 창조하며, 분석가의 역전이를 세심하고 깊이 자가-분석하여 이를 환자의 무의식을 이해하는데 활용한다. 분석가 자신 또한 분석가의 마음속에서 분석의 대상이 되어야하는 것이다. 그런데 자신을 분석하는 GPT 챗봇이라는 것이 가능할까? 그 이전에 무의식과 감정을가지는 GPT 챗봇이 가능한 걸까? 많은 SF 영화 (Bicentennial men, Her, AI 등)에서 인공지능을 가진 로봇이 감정을 가지게 되어서 사람과 똑같은 감정을 느끼게 되는 서사를 다루고 있다. 그러나 이것이 진정한 감정인지, 인간의 감정에 가장 확률적으로 최적합하게 반응하는것인지 그것을 구별할 수 있을까? 환자가 챗봇에게 진정한 신뢰와 의지를 할 수 있는지, 그리고 챗봇과의 치료관계를 통해 진정한 헤어짐, 독립을 원할 수 있게 될 수 있는지, 이러한 질문에 대한 논의도 필요할 것이다. 4. 지금까지 살펴본 것과 같이, GPT 챗봇이 분석가 혹은 심리상담사의 역할을 대신할 경우 큰 한계점이 있지만, 무시 못할 장점도 있다고 생각한다. 우선 개개인에 맞춰진 것이 아닌 일반적인 GPT를 활용한다면 매우 저렴한 비용으로 환자들이 일반적인 상담을 받을 수 있다. 정신분석이 아니라, 지지정신치료나 인지치료는 어쩌면 GPT 챗봇이 어느 정도는 대신 할 수도 있으리라고 생각한다. 지역사회나 국가의 정신보건 관리 차원에서 일차적인 예방 및 치료 접근법으로 고려할 수 있다. 또 GPT 챗봇이 감정이 없다는 것이 장점이 될 수 있다. 환자의 도발과 공격에도 감정적인 손상을입고 반사적인 반응을 하지 않기에, 어떤 면에서는 안전하고 일관적인 태도로 반응할 수 있다. 하지만 이런 면도 꼭 장점만 아니다. 기계적으로 일관된 태도는 환자에게 거절감과 무력감, 단절감을 느끼게 할 수 있기 때문이다. 또 다른 장점은 치료전문가를 찾기 힘든 지역의 환자들에게도 차선의 대안이 될 수 있는 것이다. 5. GPT 기술이 인간의 능력을 대체할 가능성이 커지면서, 사람만이 할 수 있는 것이 무엇인가하는 질문을 하게 되는 것처럼, 우리 분석가들에게도 그런 질문이 필요해졌다. 이 논의를 통해 현대와 미래의 정신분석가에게 요구되는 것이 무엇인가에 대해 더 깊이 성찰해보는 기회가 될 것이다. 앞으로의 정신분석은 점점 더 각각 환자에게 특수하고 구체적인 서사를 함께 창조하고, 그 의미를 이해하고, 환자 삶에 자신만의 새로운 관점을 만들어갈 수 있는 과정이 될 것이다. 우리 정신분석가들은 이렇게 개별화되어가는 정신분석에서 공통된 가치와 근본적인 개념을 어떻게 지킬 것인가하는질문에 답할 준비를 해야 한다. 즉, 과거와 현재, 미래, 또 전통과 혁신을 어떻게 통합하고 조화를 이루는 것이 필요하다.
